摘 要:目的:探讨增液通腑逐瘀方对脓毒症大鼠肠道免疫失衡和菌群紊乱的调节作用。方法:40只SPF级雄性SD大鼠适应性喂养1周,采用随机数字表法分为假手术组、模型组及增液通腑逐瘀方低剂量组、增液通腑逐瘀方高剂量组,每组各10只。除假手术组外,其余组大鼠采用盲肠结扎穿孔术建立脓毒症模型。增液通腑逐瘀方低剂量组(0.5 g·mL^(-1))、增液通腑逐瘀方高剂量组(1 g·mL^(-1))大鼠于术前开始以5 mL·kg^(-1)剂量灌胃给予增液通腑逐瘀方溶液,模型组及假手术组给予等体积生理盐水灌胃,每天1次。手术72 h后,观察各组大鼠的生存情况,记录生存数量;HE染色评估肠黏膜损伤程度;流式细胞术检测各组大鼠结肠固有层辅助性T细胞17(T helper cell 17,Th17)、调节性T细胞(regulatory T cell,Treg)的细胞比例;取大鼠肠道粪便进行菌落分析,并考察Th17/Treg和厌氧菌/需氧菌之间的关系。结果:与假手术组比较,其余各组脓毒症大鼠的生存数量明显降低,回肠隐窝深度、绒毛高度、绒毛表面积及黏膜厚度显著降低,Chiu′s评分显著升高,结肠Th17细胞比例、Th17/Treg比值明显升高,Treg细胞比例明显降低,肠道双歧杆菌、乳酸杆菌、厌氧菌/需氧菌比例显著降低,大肠杆菌、肠球菌比例显著升高,差异均有统计学意义(P<0.05)。与模型组比较,增液通腑逐瘀方低、高剂量组大鼠72 h生存数量明显增多;肠隐窝深度、绒毛高度、绒毛表面积及黏膜厚度明显升高,Chiu′s评分显著降低,肠黏膜损伤明显改善;Th17细胞、Th17/Treg比值明显降低,Treg细胞明显升高;双歧杆菌、乳酸菌及厌氧菌/需氧菌比例显著升高,大肠杆菌及肠球菌含量显著降低,差异均有统计学意义(P<0.05);Spearman相关分析发现,Th17/Treg比例与厌氧菌/需氧菌比例存在明显负相关关系(rs<0,P<0.05)。结论:增液通腑逐瘀方可修复脓毒症导致的肠黏膜损伤,调节肠道菌群�Objective:To probe into the regulating effect of Zengye Tongfu Zhuyu Decoction(增液通腑逐瘀方,ZYTFZYD)on intestinal immune imbalance and flora disorder in septic rats.Methods:A total of 40 SPF-class male SD rats were adaptively fed for one week.Then they were randomly divided into the sham operation group,the model group,the low-dose group of ZYTFZYD and the high-dose group of ZYTFZYD,with 10 rats in each group.Except the rats of the normal group,the rats in other groups were treated with cecal ligation and perforation to establish sepsis model.The rats in the low-dose group of ZYTFZYD(0.5 g·mL^(-1))and the high-dose group of ZYTFZYD(1 g·mL^(-1))were given ZYTFZYD at the dose of 5 mL·kg^(-1) before operation,and the model group and the sham operation group were given the same volume of normal saline once a day.Seventy-two hours after operation,the survival of rats in each group was observed and the survival number was recorded.HE staining was used to evaluate the degree of intestinal mucosal injury.The proportion of T helper cell 17(Th17)and regulatory T cell(Treg)in colonic lamina propria of rats in each group was detected by flow cytometry.Rat intestinal feces were taken for colony analysis,and the relationship between Th17/Treg and anaerobic/aerobic bacteria was analyzed.Results:Compared with the sham operation group,the survival number of septic rats in other groups decreased clealy;the depth of ileal recess,villus height,villus surface area and mucosal thickness decreased significantly;Chiu′s score increased significantly;the proportion of colon Th17 cells and Th17/Treg ratio increased clearly;the proportion of Treg cells decreased clearly;the proportion of intestinal bifidobacteria,Lactobacillus,anaerobic bacteria/aerobic bacteria decreased significantly,and the proportion of escherichia coli and enterococcus increased significantly.All the differences were statistically significant(P<0.05).Compared with the model group,the 72-hour survival number of rats in the low-dose and high-dose groups of
