平肝潜阳通络汤联合头针治疗脑梗死后遗症临床研究

摘　　要：目的:观察平肝潜阳通络汤联合头针治疗脑梗死后遗症的临床疗效。方法:将76例脑梗死后遗症患者按照随机数字表法分为观察组和对照组,每组各38例。对照组给予阿司匹林肠溶片、瑞舒伐他汀钙片治疗,观察组在对照组治疗的基础上给予平肝潜阳通络汤联合头针治疗。比较两组患者的临床疗效及治疗前后临床症状积分、美国国立卫生研究院卒中量表(national institutes of health stroke scale,NIHSS)评分、简易智能精神状态量表(mini-mental state examination,MMSE)评分、睡眠质量[匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PQSI)评分、改良Fugl-Meyer量表(Fugl-Meyer assessment scale,FAM)]评分及脑血流动力学[血管阻力指数(resistance Index,RI)、脑血流峰值时间(time topeak,TTP)、双侧大脑动脉平均血流流速(mean flow velocity,Vm)、脑平均血流量(mean blood flow,Qm)]变化情况。结果:观察组有效率为94.74%,对照组有效率为68.42%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后各项临床症状积分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后NIHSS、PSQI评分低于本组治疗前,且治疗后观察组低于对照组;两组患者治疗后MMSE、FAM评分高于本组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组患者治疗后RI、TTP低于本组治疗前,治疗后观察组低于对照组;两组患者治疗后Vm、Qm高于本组治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。结论:平肝潜阳通络汤联合头针治疗脑梗死后遗症,可改善患者神经功能、精神状态、睡眠质量及肢体功能,亦可调节脑部血流状态。Objective:To observe the clinical efficacy of Decoction for Pacifying the Liver and Subduing Yang and Dredging Meridians combined with scalp acupuncture on sequelae of cerebral infarction.Methods:A total of 76 patients with sequelae of cerebral infarction were randomly divided into the observation group and the control group,with 38 cases in each group.The control group was given Aspirin Enteric Coated Tablets and Rosuvastatin Calcium Tablets orally,while the observation group was treated with Decoction for Pacifying the Liver and Subduing Yang and Dredging Meridians combined with scalp acupuncture on the basis of the treatment of the control group.The clinical efficacy,the changes of clinical symptom scores,the scores of National Institutes of Health Stroke Scale(NIHSS),Mini Mental State Examination(MMSE)and sleep quality[including Pittsburgh sleep quality index(PSQI)score,limb modified Fugl Meyer Assessment Scale(FAM)score]and cerebral hemodynamics[including vascular resistance index(RI),time peak(TTP),mean flow velocity(VM)and mean blood flow(QM)of bilateral large cerebral arteries]before and after treatment of the two groups were compared.Results:The effective rate was 94.74%in the observation group and 68.42%in the control group.There was significant difference between the two groups(P<0.05).The clinical symptom scores of the two groups after treatment were lower than those before treatment,and the clinical symptom score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The NIHSS and PSQI scores of the two groups after treatment were lower than those before treatment,and those indexes of the observation group after treatment were lower than those of the control group;The MMSE and FAM scores of the two groups after treatment were higher than those before treatment,and those indexes of the observation group were higher than those of the control group,and all the differences were statistically significant(P<0.05).The RI and TTP of the two
