醒脑开窍针刺法与高压氧对缺血缺氧性脑病患者疗效分析

摘　　要：目的 分析醒脑开窍针刺法与高压氧治疗缺血缺氧性脑病(HIE)的临床疗效。方法 回顾性选取2011年7月至2020年7月间广州市花都区人民医院重症医学科住院治疗的HIE患者120例,根据治疗方案的不同分为两组,每组各60例。参照组采纳高压氧治疗,试验组在参照组基础上采纳醒脑开窍针刺法治疗,两组均在治疗7d后评价治疗效果,比较两组的临床疗效、临床症状改善情况、血清炎症因子、血清NSE、BDNF因子、不良反应发生率。结果 试验组临床总有效率(96.67%)高于参照组(76.67%),试验组肌张力改善、原始反射改善、惊厥停止时间均短于参照组,试验组治疗7 d后血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C-反应蛋白(hs-CRP)、神经元特异性烯醇化酶(NSE)因子均低于参照组,血清脑源性神经营养因子(BDNF)高于参照组,差异均有统计学意义(P<0.05)。试验组不良反应发生率(3.33%)与参照组(6.67%)比较,差异无统计学意义(P>0.05)。结论 醒脑开窍针刺法联合高压氧可有效改善HIE患者原始反射、肌张力等,减轻炎症反应,且不良反应较少,安全性较高,值得借鉴。Objective To analyze the clinical efficacy of Xingnao Kaiqiao acupuncture and hyperbaric oxygen in treating hypoxic-ischemic encephalopathy (HIE). Methods A total of 120 patients with HIE hospitalized in the Department of Intensive Care Medicine in Huadu District People′s Hospital of Guangzhou from July 2011 to July 2020 were selected and grouped according to different treatment regimens, with 60 cases in each group. The reference group was treated with hyperbaric oxygen, and the experimental group was treated with Xingnao Kaiqiao acupuncture based on the treatment of the reference group. The therapeutic effect was evaluated after seven days of treatment in both groups. The clinical efficacy, improvement of clinical symptoms, serum inflammatory factors, serum NSE, BDNF factors, and incidence rate of adverse reactions were compared between the two groups. Results The overall response rate in the experimental group (96.67%) was higher than that in the reference group (76.67%). The time for muscle tension and primitive reflex improve and convulsion stopping in the experimental group were shorter than the reference group. The serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP), and neuron-specific enolase (NSE) factors in the experimental group were lower than those in the reference group after seven days of treatment. The serum brain-derived neurotrophic factor (BDNF) factor was higher than the reference group. The differences were statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the experimental group (3.33%) and the reference group (6.67%) (P>0.05). Conclusion Xingnao Kaiqiao acupuncture combined with hyperbaric oxygen can effectively improve the primitive reflex and muscle tension of HIE patients, reduce the inflammatory response, with fewer adverse reactions and higher safety, which is worthy of reference.
