摘　　要：目的 探讨吴茱萸穴位贴敷联合营养支持对肺癌化疗后腹泻患者的营养状况及胃肠黏膜屏障功能的影响。方法 选择2019年1月至2020年12月杭州市中医院肿瘤科门诊就诊的肺癌化疗后腹泻患者84例,随机分为两组,每组各42例。两组予补液、益生菌及纠正电解质酸碱平衡紊乱等基础治疗。对照组予以营养支持,观察组在对照组治疗基础上加吴茱萸穴位贴敷治疗。观察两组治疗前后营养状况指标[血红蛋白(Hb)和白蛋白(ALB)]和肠黏膜受损指标[血清降钙素原(PCT)]的变化,并比较临床疗效。结果 治疗1周后,两组血清Hb水平及ALB水平明显上升(P<0.05或P<0.01),且观察组上升程度较对照组更显著(P<0.05);两组血清PCT水平较前明显下降(P<0.05或P<0.01),且观察组下降程度较对照组更显著(P<0.05);同时观察组临床总有效率(95.24%)较对照组(80.95%)更佳(χ^(2)=4.091,P<0.05)。结论 吴茱萸穴位贴敷联合营养支持治疗肺癌化疗后腹泻患者的疗效确切,不仅可提升其营养状况,减少营养不良发生,而且可保护胃肠黏膜屏障,改善胃肠道功能。Objective To investigate the effect of acupoint application of Evodia rutaecarpa combined with nutritional support on nutritional status and gastrointestinal mucosal barrier function in patients with diarrhea after chemotherapy for lung cancer.Methods From January 2019 to December 2020,a total of 84 patients with diarrhea after chemotherapy for lung cancer admitted to the Outpatient Department of Oncology in Hangzhou Hospital of Traditional Chinese Medicine were randomly divided into two groups,with 42 cases in each group.The two groups were given basic treatment such as fluid replacement,probiotics and correction of electrolyte acid-base balance disorders.The control group was given nutritional support,and the control group was treated with acupoint application of Evodia rutaecarpa based on the treatment of the control group.The changes of nutritional status indexes(hemoglobin,Hb and albumin,ALB)and intestinal mucosal damage indexes(serum procalcitonin,PCT)before and after treatment were observed,and the clinical efficacy was compared between the two groups.Results After treatment for one week,the serum Hb level and ALB level in the two groups were significantly increased(P<0.05 or P<0.01),and the increase in the observation group was more significant than that in the control group(P<0.05).The serum PCT level in the two groups was significantly decreased than before(P<0.05 or P<0.01),and the decrease in the observation group was more significant than that in the control group(P<0.05).Meanwhile,the overall clinical response rate(95.24%)in the observation group was better than that in the control group(80.95%)(χ^(2)=4.091,P<0.05).Conclusion Acupoint application of Evodia rutaecarpa combined with nutritional support is effective in treating diarrhea after chemotherapy for lung cancer.It can improve the nutritional status and reduce the occurrence of malnutrition and protect the gastrointestinal mucosal barrier and improve gastrointestinal function.
