超声造影参数联合血清微小核糖核酸-206水平在激素敏感性转移性前列腺癌患者新辅助化疗效果评估中的应用

摘　　要：目的分析超声造影参数联合血清微小核糖核酸(miR)-206水平在激素敏感性转移性前列腺癌(mHSPC)患者行新辅助化疗效果评估中的应用价值。方法选取2020年1月至2021年1月新疆维吾尔自治区人民医院收治的100例mHSPC患者作为研究对象,根据治疗效果分为治疗无效组(n=68)和治疗有效组(n=32)。全部患者均接受新辅助化疗,以21d为1个周期,共治疗6个周期。全部患者于化疗结束当天接受超声造影检查,并检测血清miR-206水平,分析mHSPC患者超声造影参数联合血清miR-206水平评估其行新辅助化疗临床效果的价值。结果100例接受新辅助化疗的mHSPC患者中,有68例患者治疗无效,治疗无效率为68%。治疗无效组的血清miR-206水平低于治疗有效组,而前列腺特异抗原(PSA)、曲线下面积(AUC)、峰值强度(PI)、强度降半时间(HT)、平均通过时间(MTT)、上升支斜率(WIS)均高于治疗有效组,差异具有统计学意义(P<0.05)。受试者工作特征(ROC)曲线结果显示,mHSPC患者化疗结束当天的超声参数联合血清miR-206水平预测新辅助化疗无效的AUC为0.830,有一定评估价值。结论超声造影参数联合血清miR-206水平评估mHSPC患者新辅助化疗效果的应用价值较高。Objective To analyze the application value of contrast-enhanced ultrasound parameters combined with serum microRNA(miR)-206 level in the evaluation of neoadjuvant chemotherapy in patients with metastatic hormone sensitive prostate cancer(mHSPC).Methods 100 mHSPC patients admitted to People′s Hospital of Xinjiang Uygur Autonomous Region from January 2020 to January 2021 were selected as the research subjects.According to the treatment effect,they were divided into ineffective group(n=68)and effective group(n=32).All patients received neoadjuvant chemotherapy,with 21 days as a cycle,a total of 6 cycles of treatment.On the day of the end of chemotherapy,all patients received contrast-enhanced ultrasound examination,serum miR-206 level was detected,and the value of mHSPC patients′contrast-enhanced ultrasound parameters combined with serum miR-206 level was analyzed to evaluate the clinical efficacy of neoadjuvant chemotherapy.Results Among 100 mHSPC patients receiving neoadjuvant chemotherapy,68 patients were ineffective,and the ineffective rate was 68%.Serum miR-206 level in the inefective group were lower than that in the effective group,and prostate specific antigen(PSA),area under the curve(AUC),peak intensity(PI),half time of intensity drop(HT),mean through time(MTT)and the wash in slop(WIS)in the ineffective group were higher than those in the effective group(P<0.05).The results receiver operating characteristic(ROC)curve showed that the AUC of ultrasound parameters combined with serum miR-206 on the day of the end of chemotherapy in predicting the risk of ineffective neoadjuvant chemotherapy in mHSPC patients was 0.830,which had certain evaluation value.Conclusions Contrast-enhanced ultrasound parameters combined with serum miR-206 level has high value in evaluating the effect of neoadjuvant chemotherapy in mHSPC patients.
