前列舒通联合非那雄胺对良性前列腺增生术后患者血管内皮生长因子、热休克蛋白、前列腺特异抗原及性激素的影响分析

摘　　要：目的探讨前列舒通联合非那雄胺治疗良性前列腺增生(BPH)术后患者的临床效果。方法选取2016年1月至2019年12月蚌埠市第一人民医院诊治的80例BPH术后患者作为研究对象。根据治疗方案的不同分为单药组(n=50)和联合组(n=30)。单药组采用非那雄胺治疗,联合组采用前列舒通联合非那雄胺治疗。观察并比较两组临床疗效、治疗前后尿动力学指标[膀胱残余尿量(RV)、最大尿流量(Qmax)]、性激素指标以及血管内皮生长因子(VEGF)、热休克蛋白(HSP)、前列腺特异抗原(PSA)水平和不良反应。结果联合组总有效率(93.33%)高于单药组(76.00%),差异具有统计学意义(P<0.05);治疗4周后,联合组RV小于单药组,Qmax大于单药组,差异具有统计学意义(P<0.05);治疗4周后,联合组雌二醇水平高于单药组,差异具有统计学意义(P<0.05);治疗4周后,联合组VEGF、HSP及PSA水平低于单药组,差异具有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论前列舒通联合非那雄胺治疗BPH的临床效果确切,可有效改善患者尿动力学指标,调节性激素水平,降低VEGF、HSP及PSA水平,且具有安全性。Objective To explore the value of Qianlieshutong combined with finasteride in treatment of patients with benign prostatic hyperplasia(BPH)after operation.Methods 80 BPH patients treated in Bengbu First People′s Hospital from January 2016 to December 2019 were selected as the study subjects.According to different treatment regimens,they were divided into single-agent group(n=50)and combination group(n=30).Monotherapy group was treated with finasteride,and combination group was treated with Qianlieshutong combined with finasteride.Clinical efficacy,urinary dynamics indexes[residual bladder volume(RV),maximum urine flow(Qmax)],sex hormone indexes,vascular endothelial growth factor(VEGF),heat shock protein(HSP),prostate specific antigen(PSA)levels and adverse reactions were observed and compared between the two groups before and after treatment.Results The total effective rate of the combined group(93.33%)was higher than that of the single-agent group(76.00%),and the difference was statistically significant(P<0.05).After 4 weeks of treatment,the RV of the combined group was smaller than that of the single-agent group,and the Qmax was larger than that of the single-agent group(P<0.05).The estradiol level of the combined group was higher than that of the single-agent group(P<0.05).The levels of VEGF,HSP and PSA in the combination group were lower than those in the single-agent group(P<0.05).The difference in the incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusions Qianlieshutong combined with finasteride in the treatment of BPH can effectively improve the patient′s urodynamics,regulate the level of sex hormone,reduce the level of VEGF,HSP and PSA,improve the treatment effect of the disease,and has safety.
