摘　　要：目的研究老年前列腺癌患者组织B淋巴细胞瘤-2蛋白(Bcl-2)、细胞FLICE样抑制蛋白(cFLIP)表达水平与临床病理特征及预后的关系。方法选取2016年6月至2019年6月庐江县人民医院收治的93例行前列腺癌根治术的老年患者作为研究对象。分别留取其的肿瘤组织、癌旁组织及正常前列腺组织标本,采用免疫组织化学法检测3种组织中Bcl-2、cFLIP蛋白表达水平,结合患者临床病理特征进行分析,采用Kaplan-Meier生存曲线对患者24个月的生存情况进行比较。结果肿瘤组织中Bcl-2、cFLIP阳性表达率分别为73.12%和68.82%,与癌旁组织和正常前列腺组织比较,差异具有统计学意义(P<0.05);肿瘤组织Bcl-2、cFLIP阳性率与患者年龄、术前前列腺特异抗原水平无相关性(P>0.05),但与患者Gleason评分、肿瘤TNM分期,以及淋巴结转移、骨转移相关(P<0.05);前列腺癌组织Bcl-2及cFLIP蛋白阳性表达患者24个月生存率明显低于阴性表达患者(P<0.05)。结论Bcl-2、cFLIP蛋白在老年前列腺癌患者组织中高表达,且与患者临床病理特征和预后相关,可能作为老年前列腺癌的治疗靶点。Objective To study the relationship between the expression levels of B cell lymphoma-2(Bcl-2)and cellular FLICE-like inhibitory protein(cFLIP)and clinicopathological features and prognosis in elderly patients with prostate cancer.Methods 93 elderly patients who underwent radical prostatectomy in Lujiang County People′s Hospital from June 2016 to June 2019 were selected as the research objects.Immunohistochemistry was used to detect the protein expression levels of Bcl-2 and cFLIP in the tumor tissues,paracancer tissues and normal prostate tissues,and the clinical and pathological characteristics of the patients were analyzed.Kaplan-meier survival curve was used to compare the survival of the patients at 24 months.Results The positive expression rates of Bcl-2 and cFLIP in tumor tissues were 73.12%and 68.82%,respectively,which was significantly different with that in paracancer tissues and normal prostate tissues(P<0.05).The positive rates of Bcl-2 and cFLIP in tumor tissues were not correlated with patients′age and preoperative prostate specific antigen level(P>0.05),but were correlated with Gleason score,tumor TNM stage,lymph node metastasis and bone metastasis(P<0.05).The 24 months survival rate of patients with positive expression of Bcl-2 and cFLIP protein was significantly lower than that of patients with negative expression(P<0.05).Conclusions Bcl-2 and cFLIP proteins are highly expressed in the tissues of elderly prostate cancer patients,and are related to the clinicopathological characteristics and prognosis of patients,and may play a role as therapeutic targets for elderly prostate cancer.
