悬吊线法经脐单孔腹腔镜手术在卵巢囊肿剥除中的临床应用

摘　　要：目的探讨悬吊线法经脐单孔腹腔镜手术(TU-LESS)在卵巢囊肿剥除中的临床应用效果。方法选取2019年1月至2020年12月河池市人民医院收治的90例卵巢良性肿瘤患者作为研究对象。采用对比研究方法将其分为研究组、对照A组和对照B组,各30例,所有患者均于腹腔镜下行卵巢剔除术。研究组采用悬吊线法TU-LESS,对照A组采用TU-LESS,对照B组采用传统3孔腹腔镜手术。比较三组的围术期相关指标[手术时间、术中出血量、术后血红蛋白(Hb)下降幅度、术后24h体温超过正常占比、住院时间、术中囊肿破裂率]、术后视觉模拟评分法(VAS)评分、切口美观满意度评分及并发症发生率。结果三组的手术时间、术中出血量、术后Hb下降幅度及术后24h体温超过正常占比比较,差异无统计学意义(P>0.05);三组的住院时间、术中囊肿破裂率比较,差异具有统计学意义(P<0.05);研究组术后1d、术后2d的VAS评分均低于对照A组和对照B组,差异具有统计学意义(P<0.05);研究组与对照A组切口美观满意度评分较对照B组高,差异具有统计学意义(P<0.05);三组并发症发生率比较,差异具有统计学意义(P<0.05)。结论悬吊线法TU-LESS手术效果显著,可有效暴露卵巢囊肿,减少囊肿破裂率,且切口更美观,安全可靠。Objective To explore the clinical application of suspended line method in transumbilical laparoscopic single-port surgery(TU-LESS)in ovarian cystectomy.Methods Ninety patients with benign ovarian tumors admitted to the Department of Gynecology,Hechi People′s Hospital from January 2019 to December 2020 were selected as the research objects.Using comparative research methods,they were divided into three groups:study group,control group A and control group B,with 30 cases in each group.All patients underwent laparoscopic ovarian removal.Study group was treated with suspended line method in TU-LESS,control group A was treated with TU-LESS and control group B was treated with traditional 3-port laparoscopic method.The perioperative related indexes[operating time,intraoperative blood loss,postoperative hemoglobin(Hb)decrease,proportion of 24 h postoperative body temperature exceeding normal after operation,hospital stay,intraoperative cyst rupture rate],postoperative visual analogue scale(VAS)score,incision aesthetic satisfaction score and complication rate were compared among the three group.Results There was no statistical difference between the three groups in operation time,intraoperative blood loss,postoperative Hb decrease,and proportion of 24 h postoperative body temperature exceeding normal after operation(P>0.05).There were statistical differences in the hospital stay and intraoperative cyst rupture rate among the three groups(P<0.05).The VAS scores of the study group 1 d and 2 d after the operation were lower than those of the control group A and the control group B,with a statistical difference(P<0.05).The study group and control group A had a higher incision aesthetic satisfaction score than the control group B(P<0.05).There were statistically significant differences in complication rate among the three groups(P<0.05).Conclusions The effect of suspended line method in TU-LESS is remarkable.It can effectively expose ovarian cyst,reduce cyst rupture rate.The incision is more beautiful,safe and reliable.
