摘　　要：目的观察微创内固定钢板治疗胫骨平台骨折的疗效。方法回顾性分析2018年9月至2019年9月在浙江省台州医院住院治疗的胫骨平台骨折患者80例,根据手术方法不同分为观察组和对照组,每组各40例,对照组行交锁髓内钉内固定术,观察组行微创内固定钢板治疗,比较两组患者的膝关节优良率、骨折愈合时间及VAS评分术前及术后变化情况。结果所有患者均获随访,随访时间6~18个月,入选患者切口均愈合。膝关节疼痛视觉模拟量表评分术后较术前明显降低。观察组患者的优良率为85.00%,明显高于对照组的67.50%,差异有统计学意义(P<0.05)。观察组患者切口愈合时间明显短于对照组,VAS评分术后较术前明显降低,且观察组患者术后VAS评分明显低于对照组,差异有统计学意义(P<0.01)。结论微创内固定钢板治疗胫骨平台骨折的疗效确切,安全性好,患者疼痛程度轻、骨折愈合时间快、术后恢复快,值得临床广泛推广和应用。Objective To observe the curative effect of minimally invasive internal fixation of steel plate in the treatment of tibial plateau fractures.Methods A total of 80 patients with tibial plateau fractures who were hospitalized in the Department of Internal Medicine of our hospital from September 2018 to September 2019 were retrospectively analyzed.According to different surgical methods,they were divided into an observation group and a control group,with 40 cases in each group.The control group was given interlocking intramedullary nail internal fixation,and the observation group was given minimally invasive internal fixation of steel plate.The excellent and good rate of knee joints,fracture healing time,and changes of VAS score before and after operation were compared between the two groups.Results All patients were followed up for 6 to 18 months.The incisions of the selected patients were all recovered.The visual analog scale score of knee joint pain was significantly lower after operation than before operation.The excellent and good rate of patients in the observation group was 85.00%,which was significantly higher than that in the control group(67.50%)(P<0.05).The incision healing time in the observation group was significantly shorter than that in the control group.The VAS score was significantly lower after surgery than before surgery.Moreover,the postoperative VAS score in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.01).Conclusion The minimally invasive internal fixation of steel plate is effective and safe in the treatment of tibial plateau fractures.Patients show mild pain,fast fracture healing time,and quick postoperative recovery.This method is worthy of widespread clinical promotion and application.
