摘　　要：目的观察舒郁定悸汤治疗气滞血瘀证持续性心房颤动伴抑郁状态的疗效。方法依据随机数字表法将2018年1月至2020年9月舟山市中医院心血管内科住院及门诊治疗的持续性心房颤动伴抑郁状态患者62例分为治疗组与对照组,每组各31例。两组均采用常规西药治疗,对照组在常规西药基础上加用黛力新片治疗,治疗组在对照组基础方案上加用舒郁定悸汤口服治疗,观察周期均8周。评估两组治疗前后的中医症候积分、24 h动态心电图平均心室率及汉密尔顿抑郁量表(hamilton depression scale,HAMD)评分变化情况。结果治疗前后比较,两组患者的平均心室率和HAMD评分均有所下降,治疗组的平均心室率和HAMD评分明显低于对照组,差异有统计学意义(P<0.05)。治疗组的中医症侯疗效总有效率持续性心房颤动及抑郁疗效总有效率均高于对照组(P<0.05)。结论舒郁定悸汤治疗气滞血瘀证持续性心房颤动伴抑郁状态临床疗效确切,能缓解临床症状,控制心室率,减轻抑郁情绪,值得临床推广。Objective To observe the curative effect of Shuyu Dingji Decoction in treating persistent atrial fibrillation with depression of Qi stagnation and blood stasis syndrome.Methods According to the random number table,62 patients with persistent atrial fibrillation and depression from the inpatient and outpatient clinic of the Department of Cardiology in Zhoushan Hospital of Traditional Chinese Medicine from January 2018 to September 2020 were divided into treatment group and control group,with 31 cases in each group.Both groups were treated with conventional western medicine.The control group was treated with Dailixin tablets on the basis of conventional western medicine treatment,and the treatment group was treated with Shuyu Dingji Decoction on the basis of the treatment of the control group.The observation period lasted for 8 weeks.The scores of traditional Chinese medicine syndrome,24-hour Holter average ventricular rate and hamilton depression scale(HAMD)score were assessed before and after treatment in the two groups.Results After treatment,average ventricular rate and HAMD score of the two groups of patients decreased.The effect of the treatment group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).The total effective rate of TCM symptonios,persistert artrial fibrilllation and efficacy of depession were all higher than the control group(P<0.05).Conclusion Shuyu Dingji Decoction has a definite clinical effect in treating persistent atrial fibrillation with depression of Qi stagnation and blood stasis syndrome.It can relieve clinical symptoms,control ventricular rate,and reduce depression.It is worthy of clinical promotion.
