摘  要:目的 探讨电针激痛点对慢性肌筋膜疼痛综合征(myofascial pain syndrome,MPS)大鼠L4~6脊髓背角星形胶质细胞神经胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)和小胶质细胞特异表达补体受体-3的单克隆抗体(OX-42)表达的影响。方法 将24只雄性SD大鼠随机分为正常组、模型组、电针组,每组8只。采用打击结合离心运动方式建立MPS模型。电针组选取激痛点,电针15 min/次,1次/d,治疗7 d。采用HE染色法观察大鼠激痛点处肌肉组织形态学变化;肌电图仪检测大鼠激痛点处自发电位变化;免疫组化法检测大鼠L4~6脊髓背角GFAP及OX-42的阳性细胞表达。结果 与正常组相比,模型组大鼠自发电位活动频率显著增多(P<0.05);与模型组相比,电针组自发电频率显著下降(P<0.05)。与正常组比较,模型组GFAP、OX-42阳性细胞表达增多(P<0.05);与模型组比较,电针组GFAP、OX-42阳性细胞表达减少(P<0.05)。结论 电针激痛点可降低MPS大鼠激痛点处自发电活动频率,抑制脊髓背角GFAP、OX-42阳性细胞过表达,其机制或与调节中枢敏化密切相关。Objective To investigate the effects of electroacupuncture trigger points on the expression of astrocyte glial fibrillary acidic protein(GFAP)and microglia-specific expressing complement receptor-3 monoclonal antibody(OX-42)in L4~6 spinal cord dorsal horn in rats with chronic myofascial pain syndrome(MPS).Methods Twenty-four male SD rats were randomly divided into normal group,model group and electroacupuncture group,with 8 rats in each group.The MPS model was established by strike combined with centrifugal motion.In the electroacupuncture group,trigger points were selected and electroacupuncture was performed for 15 minutes each time,once a day,for 7 days.HE staining was used to observe the morphological changes of muscle tissue at the trigger points.Electromyography(EMG)was used to detect the spontaneous potential changes.Immunohistochemistry was used to detect the expression of GFAP and OX-42 in L4~6 spinal dorsal horn of rats.Results Compared with the normal group,the frequency of spontaneous potential activity in the model group was significantly increased(P<0.05).Compared with the model group,the spontaneous electric frequency of the electroacupuncture group was significantly decreased(P<0.05).Compared with the normal group,the expression of GFAP and OX-42 positive cells in the model group were increased(P<0.05).Compared with the model group,the expressions of GFAP and OX-42 positive cells in the electroacupuncture group were decreased(P<0.05).Conclusion Electroacupuncture trigger points could effectively reduce the spontaneous electrical activity frequency in MPS rats,and inhibit the overexpression of GFAP and OX-42 positive cells in the dorsal horn of spinal cord.And its mechanism may be closely related to the regulation of central sensitization.
