摘　　要：目的探讨龙胆泻肝汤联合刺血疗法治疗带状疱疹肝经郁热证的临床疗效。方法选取60例肝经郁热证带状疱疹患者,以随机数表法分为对照组与观察组,各30例。对照组予维生素B;片、甲钴胺片、盐酸伐昔洛韦片治疗,观察组在对照组基础上予龙胆泻肝汤联合刺血疗法治疗,2组均持续治疗10 d。比较2组治疗后临床疗效,治疗前后疼痛程度、外周血T淋巴细胞亚群水平、血清疼痛介质及炎性因子水平。结果治疗后,观察组总有效率(93.33%,28/30)高于对照组(70.00%,21/30)(P<0.05)。治疗后,2组视觉模拟评分法(VAS)评分均降低,观察组低于对照组(P<0.05);2组外周血CD4^(+)、CD4^(+)/CD8^(+)及血清β-内啡肽(β-EP)水平均升高,观察组高于对照组(P<0.05);2组外周血CD8^(+)及血清环氧化酶-2(COX-2)、白介素-10(IL-10)水平均降低,观察组低于对照组(P<0.05)。结论龙胆泻肝汤联合刺血疗法可降低带状疱疹肝经郁热证患者疼痛程度,缓解机体炎症反应,调节机体T淋巴细胞亚群,提高临床疗效。Objective To explore the clinical effect of Longdan Xiegan Decoction combined with the blood pricking therapy in the treatment of herpes zoster with the syndrome of heat stagnation in liver channels.Methods A total of 60 herpes zoster patients with the syndrome of heat stagnation in liver channels were selected as the research subjects and randomly divided into a control group and an observation group,with 30 cases in each group.The control group was treated with vitamin B1 tablets,mecobalamin tablets,and valacyclovir hydrochloride tablets,while the observation group was treated with Longdan Xiegan Decoction combined with the blood pricking therapy on the basis of the control group,and both groups were treated for 10 days.The clinical efficacy,pain degree,levels of peripheral blood T lymphocyte subsets,levels of serum pain mediators and inflammatory factors before and after treatment were compared between the two groups.Results After treatment,the total effective rate in the observation group(93.33%,28/30)was higher than that in the control group(70.00%,21/30)(P<0.05).After treatment,the visual analogue scale(VAS)scores of both groups were decreased,and the observation group was lower than the control group(P<0.05).The levels of peripheral blood CD4^(+),CD4^(+)/CD8^(+)and serumβ-endorphin(β-EP)were all increased in the two groups,and the observation group was higher than the control group(P<0.05),while the levels of peripheral blood CD8^(+),serum cyclooxygenase-2(COX-2)and interleukin-10(IL-10)were decreased in the two groups,and the observation group was lower than the control group(P<0.05).Conclusion Longdan Xiegan Decoction combined with the bloodletting therapy can reduce the pain degree of herpes zoster patients with the syndrome of heat stagnation in liver channels,relieve the body’s inflammatory reactions,regulate the body’s T lymphocyte subsets,with a significant curative effect.
