时间-空间相关成像技术联合动态正交三平面分析法在胎儿主动脉缩窄筛查中的价值研究

摘  要:目的:探究时间-空间相关成像(spatio-temporal image correlation,STIC)技术联合动态正交三平面(multi-planar,MP)分析法在胎儿主动脉缩窄(coarctation of the aorta,COA)筛查中的应用。方法:选取2019年4月—2021年10月于普宁市人民医院接受胎儿超声心动图检查确诊主动脉缩窄胎儿的孕妇40例,对该40例孕妇均进行二维超声心动图检查(对照组)以及时间-空间相关成像技术联合MP分析法检查(研究组),对比两种技术检测的心脏各切面显示率和心脏畸形检出率。结果:研究组在对主动脉纵切面、三血管-气管切面、主动脉弓冠状切面显示率上均高于对照组,且两组主动脉纵切面与主动脉弓冠状切面显示率之间,差异有统计学意义(P<0.05);根据实际结果检测出该40例主动脉缩窄胎儿中,在单发畸形9例以及合并其他畸形31例(包括合并复杂先天性心脏病8例以及合并室间隔缺损23例)当中,研究组心脏畸形检出率(97.5%)显著高于对照组脏畸形检出率(85.0%),差异有统计学意义(P<0.05)。结论:应用时间-空间相关成像技术联合MP分析法能够显示胎儿心室流出道切面,使心脏大动脉连接部的显示更加定量和步骤化,减少了对操作者技术水平的依赖,有利于提高心脏各切面显示率和胎儿心脏畸形的检出率,具有潜在的临床应用价值。Objective To explore the application of spatio-temporal image correlation(STIC)technique combined with dynamic orthogonal triplane(multi-planar,MP)analysis in fetal aortic coarctation(coarctation of the aorta,COA)screening.Methods From April 2019 to October 2021,40 pregnant women with fetal echocardiography were diagnosed with COA.Two-dimensional echocardiography(control group)and STIC combined MP analysis(study group)were performed to compare the heart section display rate and cardiac malformation detection rate detected by the two techniques.Results The detection rate of aortic longitudinal section,trivascular-tracheal section and aortic arch coronal section in the study group was higher than that in the control group(P<0.05).The results showed that 9 cases of single malformation and 3 lcases of other malformations(including 8 cases with complicated congenital heart disease and 23 cases with ventricular septal defect)were detected in 40 cases of coarctation fetus,detection rate of cardiac malformations study group(97.5%)was significant higher than that in the control group(85.0%),the difference was statistically significant(P<0.05).Conclusion Application of spatio-temporal image correlation combined with MP analysis method can show the fetal ventricular outflow tract section,make the display of the connection part of the heart artery more quantitative and step-by-step,reduce the dependence on the operator’s technical level,and help to improve the display rate of each section of the heart and the detection rate of fetal heart malformation,which has potential clinical application value.
