十灰散化裁治疗混合痔术后出血疗效研究

摘　　要：目的:探讨十灰散化裁对混合痔术后出血的影响。方法:选取混合痔患者400例为观察对象,采用随机数字表法分为观察组和对照组各200例。对照组常规行混合痔外剥内扎术,术后给予酚磺乙胺片口服,观察组在对照组基础上给予十灰散化裁治疗。比较两组患者治疗后不同时间点出血症状积分、凝血功能指标、出血停止情况和临床疗效,并进行安全性分析。结果:两组术后不同时间点出血症状积分比较,差异有统计学意义(均P<0.05)。术后各时间点两组出血症状积分比较,差异有统计学意义(均P<0.05)。两组治疗前后APTT、TT、PT、Fib水平比较,差异有统计学意义(均P<0.05)。治疗后两组各指标比较,差异有统计学意义(均P<0.05)。两组术后0~2 d、6~7 d及7 d后的出血停止例数比较,差异有统计学意义(均P<0.05)。观察组临床总有效率为90.50%,对照组临床总有效率为80.00%,两组比较差异有统计学意义(P<0.05)。结论:十灰散化裁可有效减轻混合痔术后出血患者临床症状,改善患者凝血功能,缩短出血时间,疗效较好,且安全较高。Objective:To observe influence of modified Shihui powder on postoperative bleeding of mixed hemorrhoids.Methods:400 patients with mixed hemorrhoids were enrolled as the observation subjects,randomly divided into the observation group and the control group with 200 cases in each group by random number table method.After surgery,the control group was routinely treated with milligant-morgant hemorrhoidectomy of mixed hemorrhoids and given oral administration of etamsylate tablets,and the observation group was treated with modified Shihui powder on the basis of the control group.The score of bleeding symptom,coagulation function indicators,bleeding cessation and clinical efficacy were compared between the two groups at different time points after treatment,and safety was analyzed.Results:There was statistical significance in the bleeding symptom score at different time points after surgery within the two groups(all P<0.05).After surgery,there was statistical difference in the score of bleeding symptom at each time point between the two groups(all P<0.05).The differences in APTT,TT,PT and Fib were statistically significant in the two groups before and after treatment(all P<0.05).After treatment,there were statistically significant differences in the indicators between the two groups(all P<0.05).There were statistical differences in the number of patients with bleeding cessation at 0-2 d,6-7 d or more than 7 d after surgery between the two groups(all P<0.05).The total clinical effective rate was 90.50%in the observation group and that in the control group was 80.00%,difference in the total clinical effective rate between the two groups statistically significant(P<0.05).Conclusion:Modified Shihui powder can effectively relieve the bleeding symptom,improve the coagulation function,and shorten the bleeding time of patients with postoperative bleeding of mixed hemorrhoids.It has better clinical efficacy and security.
