摘 要:目的:探讨乳腺钼靶在乳腺钙化诊断中的临床准确性。方法:选取日照市中医医院2017年7月—2020年7月收治的乳腺钙化患者50例,患者均经临床病理检查确诊为乳腺钙化病变,根据病变情况分为良性钙化组(n=8,乳腺良性钙化病变)和恶性钙化组(n=42,乳腺恶性钙化病变),患者均行乳腺钼靶诊断,以病理检查结果为金标准,分析乳腺钼靶诊断乳腺钙化良、恶性病变的诊断效能,并对两种病变的病灶特征及钙化情况进行对比分析。结果:50例乳腺钙化病变患者行乳腺钼靶检查显示恶性病变为41例,良性病变为9例,乳腺钼靶诊断的准确性为94.00%,特异度为87.50%,灵敏度为95.24%,与病理诊断结果比较无显著差异(P>0.05);乳腺钼靶诊断显示,恶性钙化组患者病灶≥1cm、形态不规则、边缘模糊的检出率均显著高于良性钙化组患者(P<0.05);恶性钙化组患者钙化数量≥10个/cm^(2)、等密度钙化、节段/线性分布的检出率均显著高于良性钙化组患者(P<0.05)。结论:乳腺钼靶应用于乳腺钙化患者的诊断中具有较高应用准确性,可对乳腺良、恶性病变情况进行鉴别判定,其诊断结果与病理检查结果无显著差异,临床可推广应用。Objective To investigate the clinical accuracy of mammography in the diagnosis of breast calcification.Methods Fifty patients with breast calcification admitted to Rizhao Hospital of Traditional Chinese Medicine from July 2017 to July 2020 were selected,all of whom were confirmed as breast calcification by clinicopathological examination.They were divided into benign calcification group(n=8,benign calcification of breast) and malignant calcification group(n=42,malignant calcification of breast).All patients underwent mammography,and the diagnostic efficacy of mammography in the diagnosis of benign and malignant breast calcification lesions was analyzed using pathological examination as the gold standard,and the lesion characteristics and calcification of the two lesions were compared.Results 50 patients with breast calcification lesions underwent mammography with mammography showing 41 cases of malignant lesions,9 cases of benign lesions,94.00% accuracy,87.50% specificity and 95.24% sensitivity.There was no significant difference with pathological diagnosis(P>0.05).Breast molybdenum target diagnosis showed that the detection size of lesion≥1 cm,irregular shape and fuzzy edge in malignant calcification group were significantly higher than those in benign calcification group(P<0.05).The number of calcifications in malignant calcification group was ≥10/cm^(2).The detection rates of isodensity calcification and segmental/linear distribution were significantly higher than those in benign calcification group(P <0.05).Conclusion Mammography is applied to the diagnosis of patients with mammary gland calcification,which has high application accuracy,and can distinguish between benign and malignant breast lesions,and its diagnostic results are not significantly different from the pathological examination results,and can be promoted and applied clinically.
