5-氨基酮戊酸光动力联合火针治疗中重度痤疮的疗效分析

摘　　要：目的:探讨5-氨基酮戊酸光动力(ALA-PDT)联合火针治疗中重度痤疮的疗效以及对患者皮肤屏障功能、皮损部位炎症因子水平的影响。方法:102例中重度痤疮患者以随机抽签法分为联合组和对照组,每组51例。对照组给予ALA-PDT治疗,联合组给予ALA-PDT联合火针治疗,均治疗2个月。比较两组疗效、皮肤生理指标、皮损部位炎性因子水平,并记录不良反应情况。结果:联合组临床总有效率(90.20%)显著高于对照组(74.51%,P<0.05)。治疗后,两组皮肤角质层含水量明显升高,pH值、皮肤经皮水丢失量(TEWL)、红斑指数(EI)、黑色素指数(MI)、皮损部位肿瘤坏死因子α(TNF-α)、白细胞介素4(IL-4)、白细胞介素8(IL-8)及干扰素γ(IFN-γ)水平明显降低,且除pH值以外联合组各指标升高或降低的幅度显著高于对照组(P<0.05)。联合组治疗期间不良反应发生率(25.49%)显著低于对照组(47.09%,P<0.05)。结论:ALA-PDT联合火针治疗中重度痤疮疗效确切,可有效改善患者皮肤屏障功能,同时还能明显降低患者皮损部位炎性因子水平,该治疗方案不良反应少,具有良好的安全性,值得临床推广应用。Objective This research aims to investigate the curative effect of 5-aminolevulinic acid-mediated photodynamic therapy(ALA-PDT)combined with fi re needle in the treatment of moderate to severe acne,and the infl uence on skin barrier function and infl ammatory factor levels in skin lesions.Methods 102 patients with moderate to severe acne were randomly divided into combined group and control group,with 51 cases in each group.The control group was treated with ALA-PDT,and the combined group was treated with ALA-PDT combined with fi re needle.Both groups were given 2 months of treatment.Curative eff ect,skin physiological indicators,and infl ammatory factor levels in skin lesions were compared between the two groups,and adverse reactions were recorded.Results The total clinical response rate of the combined group(90.20%)was significantly higher than that of the control group(74.51%,P<0.05).After treatment,stratum corneum water content was significantly increased,while the pH value,transepidermal water loss(TEWL),erythema index(EI),melanin index(MI),tumor necrosis factorα(TNF-α),interleukin 4(IL-4),interleukin 8(IL-8)and interferonγ(IFN-γ)levels in skin lesions were signifi cantly reduced in the two groups.The increase or decrease of each index except pH value was signifi cantly greater in the combined group than in the control group(P<0.05).The incidence of adverse reactions during treatment was 25.49%in the combined group,which was signifi cantly lower than that in the control group(47.09%,P<0.05).Conclusion The results indicate that ALA-PDT combined with fi re needle is eff ective in the treatment of moderate to severe acne,which can eff ectively improve the patients’skin barrier function while signifi cantly reducing the levels of infl ammatory factors in skin lesions,with few adverse reactions and good safety.
