改良掌背动脉皮支筋膜蒂皮瓣修复手指近、中节皮肤软组织缺损效果分析

摘　　要：目的:分析改良掌背动脉皮支筋膜蒂皮瓣对手指近、中节皮肤软组织缺损的修复效果。方法:连续纳入2018年1月-2019年12月笔者医院收治的20例手指近、中节皮肤软组织缺损患者,缺损面积1.0 cm×2.0 cm~2.0 cm×3.0 cm,均采用改良掌背动脉皮支筋膜蒂皮瓣进行修复,皮瓣面积1.2 cm×2.2 cm~2.2 cm×3.2 cm。术后随访观察皮瓣存活及供区愈合情况,对手功能进行评价。结果:本次随访时间(7.38±1.69)个月;20例皮瓣均成活,皮瓣血运良好,皮瓣的质地及外形较好;18例供区一级愈合,2例供区再处理后愈合;皮瓣两点辨别觉距离为(7.16±1.04)mm;手功能8例优,7例良,5例可。结论:改良掌背动脉皮支筋膜蒂皮瓣对手指近、中节皮肤软组织缺损的修复效果显著,安全可靠。Objective To discuss the repair eff ect of modifi ed dorsal metacarpal artery cutaneous branch fascial pedicle fl ap in soft tissue defect of proximal and middle fi nger segments.Methods 20 patients with skin and soft tissue defect of proximal and middle fi nger segments who were continuously included in our hospital from January 2018 to December 2019,and the defect areas were ranged from 1.0 cm×2.0 cm to 2.0 cm×3.0 cm.All the patients were repaired with modifi ed dorsal metacarpal artery cutaneous branch fascial pedicle fl ap,and the fl ap areas were ranged from 1.2 cm×2.2 cm to 2.2 cm×3.2 cm.The patients were followed up after operation.The fl ap survival and donor healing was observed.The hand function was evaluated.Results The follow-up time was(7.38±1.69)months.20 cases of fl ap was survived,the blood supply of fl ap was good,and the texture and shape of fl aps was good.18 cases of donor site was primary healing,2 cases of donor was healed after reprocessing.The distance between the two-point discrimination of flap was(7.16±1.04)mm.Hand function was excellent in 8 cases,good in 7 cases and fair in 5 cases.Conclusion The modifi ed dorsal metacarpal artery cutaneous branch fascial pedicle fl ap had prominent repair eff ect in skin and soft tissue defect of proximal and middle fi nger segments,and it was safe and reliable.
