复方倍他米松注射液封闭结合中医正骨法与洛索洛芬钠片治疗腰椎小关节紊乱症的临床效果

摘　　要：目的:探讨复方倍他米松注射液封闭结合中医正骨法与洛索洛芬钠片治疗腰椎小关节紊乱症的临床效果。方法:采用随机单盲对照研究法,选取2018年1月-2020年12月北京市石景山区中医医院收治的120例腰椎小关节紊乱症患者作为研究对象。按照随机性原则将其分为对照组(60例)和研究组(60例)。对照组给予腰部斜扳法复位治疗和洛索洛芬钠片,研究组给予复方倍他米松注射液封闭结合中医正骨法与洛索洛芬钠片治疗。比较两组临床疗效,治疗前,治疗后即时、2周、1个月、2个月腰部疼痛、功能障碍,复发情况。结果:研究组总有效率高于对照组(P<0.05)。治疗后即时、2周、1个月、2个月,研究组腰部视觉模拟评分法(VAS)评分均低于对照组,腰部日本骨科协会评估治疗分数(JOA)评分均高于对照组,随访3个月,研究组复发率低于对照组(P<0.05)。结论:复方倍他米松注射液封闭结合中医正骨法与洛索洛芬钠片治疗腰椎小关节紊乱症效果显著,得宝松局部封闭辅以中医正骨法不仅能在治疗即刻就使患者获得疼痛缓解,还能有效促进腰椎功能的改善和恢复,该方法效果稳定,复发率低,值得临床推广应用。Objective:To explore the clinical effect of Compound Betamethasone Injection blocking combined with bone setting manipulation of traditional Chinese medicine and Loxoprofen Sodium Tablets in the treatment of lumbar facet joint disorder.Method:A total of 120 patients with lumbar facet joint disorders admitted to Beijing Shijingshan District Hospital of Traditional Chinese Medicine from January 2018 to December 2020 were selected as the research subjects by a randomized,single-blind controlled study method.They were divided into control group(60 cases)and study group(60 cases)according to randomness principle.The control group was given the waist oblique pulling method reduction treatment and Loxoprofen Sodium Tablets,while the study group was given Compound Betamethasone Injection blocking combined with bone setting manipulation of traditional Chinese medicine and Loxoprofen Sodium Tablets treatment.The clinical efficacy,waist pain,dysfunction before treatment,immediately after treatment,2 weeks,1 month and 2 months after treatment and recurrence condition of the two groups were compared.Result:The total effective rate of study group was higher than that of control group(P<0.05).Immediately after treatment,2 weeks,1 month and 2 months after treatment,the waist visual analogue scale(VAS)score of the study group was lower than that of the control group,and the Japanese Orthopaedic Association(JOA)score was higher than that of the control group,the recurrence rate of the study group was lower than that of the control group 3 months of follow-up(P<0.05).Conclusion:Compound Betamethasone Injection blocking combined with bone setting manipulation of traditional Chinese medicine and Loxoprofen Sodium Tablets in the treatment of lumbar facet joint disorder has a significant effect,which can promote the improvement and recovery of lumbar function,has a stable effect after treatment,and the recurrence rate is low,which is worthy of clinical promotion and application.
