摘 要:目的对比不同干预手段在糖尿病合并结核病患者治疗中的疗效。方法162例糖尿病合并结核病患者作为研究对象,按干预手段不同分为控制饮食组(55例)、口服降糖药组(55例)、胰岛素治疗组(52例)。控制饮食组患者采用饮食控制进行治疗,口服降糖药组患者采用格列齐特缓释片联合盐酸二甲双胍进行治疗,胰岛素治疗组患者采用诺和灵R进行治疗。比较三组患者痰菌转阴情况、病灶吸收情况。结果治疗1、2、6个月,控制饮食组的痰菌转阴率分别为27.27%、60.00%、78.18%,口服降糖药组分别为38.18%、69.09%、83.64%,胰岛素治疗组分别为57.69%、86.54%、96.15%。治疗1、2、6个月,胰岛素治疗组的痰菌转阴率明显高于口服降糖药组和控制饮食组,差异具有统计学意义(P<0.05)。三组治疗1、2、6个月的痰菌转阴率均呈上升趋势,组内比较差异具有统计学意义(P<0.05)。治疗1、2个月,胰岛素治疗组的病灶吸收率别为71.15%、78.85%,口服降糖药组分别为43.64%、56.36%,控制饮食组分别为32.73%、36.36%;治疗1、2个月,胰岛素治疗组的病灶吸收率高于控制饮食组和口服降糖药组,且三组病灶吸收率比较差异具有统计学意义(P<0.05)。结论通过胰岛素控制血糖可以提高肺结核合并糖尿病患者的痰菌转阴率与病灶吸收率,值得临床推广应用。Objective To compare the efficacy of different intervention methods in patients with diabetes mellitus and tuberculosis.Methods A total of 162 patients with diabetes mellitus and tuberculosis were selected as the research subjects.According to different intervention methods,they were divided into diet control group(55 cases),oral hypoglycemic drug group(55 cases),and insulin treatment group(52 cases).Patients in the diet control group were treated with diet control,patients in the oral hypoglycemic drug group were treated with gliclazide sustained-release tablets combined with metformin hydrochloride,and patients in the insulin treatment group were treated with Novolin R.The sputum negative-conversion rate and absorption of lesions the three groups were compared.Results The sputum negative-conversion rates were 27.27%,60.00%,and 78.18%in the diet control group,38.18%,69.09%,and 83.64%in the oral hypoglycemic drug group,and 57.69%,86.54%,and 96.15%in the insulin treatment group at 1,2,and 6 months of treatment,respectively.After 1,2 and 6 months of treatment,the sputum negative-conversion rates in the insulin treatment group were significantly higher than those in the oral hypoglycemic drug group and the diet control group,and the difference was statistically significant(P<0.05).The sputum negative-conversion rates of the three groups at 1,2 and 6 months of treatment showed an increasing trend,and the differences within the groups were statistically significant(P<0.05).The lesion absorption rates were 71.15%and 78.85%in the insulin treatment group,43.64%and 56.36%in the oral hypoglycemic drug group,and 32.73%and 36.36%in the diet control group at 1 and 2 months of treatment,respectively.At 1 and 2 months of treatment,the lesion absorption rates in the insulin treatment group was higher than that in the diet control group and the oral hypoglycemic drug group,and the difference in lesion absorption rate among the three groups was statistically significant(P<0.05).Conclusion Insulin control can improve the sputum neg
