摘  要:目的研究血小板与冷沉淀联合输注在外科手术中的效果及对出血时间及输血不良反应的影响。方法300例外科手术需大量输血的患者,按照输血方式不同分为观察组、对照A组、对照B组,每组100例。观察组采用血小板与冷沉淀联合输注法,对照A组采用单独血小板输注法,对照B组采用单独冷沉淀输注法。比较三组患者输血前后凝血相关指标[凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白原(FIB)]水平、出血时间和不良反应发生情况。结果输血前,三组患者的PT、APTT、TT、FIB水平比较差异均无统计学意义(P>0.05);输血后,三组患者的PT、APTT、TT、FIB水平均较输血前显著改善,观察组患者的PT(10.11±4.12)s、APTT(37.48±12.85)s、TT(13.92±3.47)s、FIB(1.99±0.45)g/L均显著优于对照A组的(18.78±4.56)s、(52.95±15.22)s、(20.86±4.11)s、(1.48±0.51)g/L和对照B组的(19.23±4.52)s、(54.29±19.53)s、(21.54±4.65)s、(1.76±0.50)g/L,差异具有统计学意义(P<0.05)。观察组、对照A组、对照B组患者的出血时间分别为(2.98±0.78)、(6.34±1.21)、(7.09±1.35)h,不良反应发生率分别为3.00%、11.00%、10.00%;观察组患者的出血时间显著短于对照A组和对照B组,不良反应发生率均低于对照A组和对照B组,差异均有统计学意义(P<0.05)。结论血小板与冷沉淀联合输注应用于外科手术中,有利于缩短患者的出血时间,降低不良反应发生率,值得临床推广应用。Objective To study the effect of platelet and cryoprecipitate combined infusion during surgery and its influence on bleeding time and adverse transfusion reaction.Methods A total of 300 patients requiring massive blood transfusion were divided into observation group,control group A,and control group B according to different blood transfusion methods,with 100 cases in each group.The observation group was treated with combined infusion of platelets and cryoprecipitate,the control group A was treated with platelet transfusion alone,and the control group B was treated with cryoprecipitate infusion alone.The levels of blood coagulation-related indexes[thrombin time(TT),activated partial thromboplastin time(APTT),prothrombin time(PT),fibrinogen(FIB)]before and after blood transfusion,bleeding time and occurrence of adverse reactions were compared among the three groups.Results Before blood transfusion,there was no statistically significant difference in the levels of PT,APTT,TT and FIB among the three groups(P>0.05).After blood transfusion,the levels of PT,APTT,TT,and FIB in the three groups were significantly improved compared with those before blood transfusion;the PT(10.11±4.12)s,APTT(37.48±12.85)s,TT(13.92±3.47)s,FIB(1.99±0.45)g/L of the observation group were significantly better than(18.78±4.56)s,(52.95±15.22)s,(20.86±4.11)s,(1.48±0.51)g/L of the control group A and(19.23±4.52)s,(54.29±19.53)s,(21.54±4.65)s,(1.76±0.50)g/L of the control group B;all the differences were statistically significant(P<0.05).The bleeding time of observation group,control group A and control group B were(2.98±0.78),(6.34±1.21),and(7.09±1.35)h,and the incidence of adverse reactions were 3.00%,11.00%,and 10.00%.The bleeding time of observation group was significantly shorter than that of control group A and control group B,and the incidence of adverse reactions was lower than that of control group A and control group B.All the differences were statistically significant(P<0.05).Conclusion The application of platelet and cryo
