厚朴排气合剂联合肠内营养对食管贲门癌手术患者肠蠕动恢复及肠道菌群的影响

摘　　要：目的:探究厚朴排气合剂联合肠内营养对食管贲门癌手术患者肠蠕动恢复及肠道菌群的影响。方法:选取2020年1月—2021年10月在该院进行手术治疗的食管贲门癌患者72例,应用随机数字表法分为观察组36例,对照组36例,对照组患者于术后给予其肠内营养治疗,观察组在对照组基础上给予患者厚朴排气合剂行治疗。比较2组患者治疗前和治疗1周后患者机体血清炎性因子水平、肠道蠕动恢复情况、腹胀情况及肠道菌群分布情况。结果:治疗后2组患者血清各炎性介质水平较治疗前均明显降低,且观察组患者血清C 反应蛋白(CRP)、降钙素原 (PCT)、白细胞水平均明显低于对照组(P<0.05);治疗后观察组规律肠鸣音恢复、肛门初次排气及首次排便时间均明显较对照组短(P<0.05);治疗后观察组与对照组腹痛发生率分别为2.78%、16.67%,观察组患者腹痛发生率明显低于对照组(P<0.05);治疗后2组患者肠道菌群水平明显优于治疗前,观察组肠道双歧杆菌、乳酸杆菌及双歧杆菌与大肠杆菌比值(B/E值)均明显高于对照组,大肠杆菌水平明显低于对照组(P<0.05)。结论:厚朴排气合剂联合肠内营养治疗对食管贲门癌术后营养状况改善效果明显,有利于机体内环境稳态调控,可显著加快肠道蠕动恢复,改善肠道内微生物菌群分布。Objective:To explore the effects of Houpu Paiqi mixture combined with enteral nutrition on the recovery of intestinal peristalsis and intestinal floras in patients undergoing esophageal cardia cancer surgery.Methods:A total of 72 patients with esophageal cardia cancer undergoing surgical treatment in the hospital were enrolled between January 2020 and October 2021.According to random number table method,they were divided into observation group (n=36) and control group (n=36).After surgery,patients in control group were given enteral nutrition,while those in observation group were additionally given Houpu Paiqi mixture.The levels of serum inflammatory factors,recovery of intestinal peristalsis,abdominal distension and distribution of intestinal floras were compared between the two groups before treatment and after 1 week of treatment.Results:After treatment,levels of serum inflammatory mediators in both groups were significantly decreased.The levels of serum C-reactive protein (CRP),procalcitonin (PCT) and white blood cells in observation group were significantly lower than those in control group (P<0.05).After treatment,recovery time of regular bowel sounds,the first anal exhaust time and the first defecation time in observation group were significantly shorter than those in control group (P<0.05).After treatment,incidence of abdominal pain in observation group was significantly lower than that in control group (2.78% vs 16.67%) (P<0.05).After treatment,levels of intestinal floras in both groups were significantly improved.The intestinal bifidobacteria,lactobacillus and ration of bifidobacteria to Escherichia coli (B/E) in observation group were significantly higher than those in control group,and level of Escherichia coli was significantly lower than that in control group (P<0.05).Conclusion:Houpu Paiqi mixture combined with enteral nutrition can significantly improve nutritional status after esophageal cardia cancer surgery,which is beneficial to regulate internal environment homeostasis,significantly accelerat
