摘 要:牙周炎是由多种牙周微生物感染引起的牙周软硬组织的慢性、进行性、不可逆性损害的炎症性疾病。抗菌药物是目前治疗牙周炎的一线用药,能够显著改善牙周组织炎症。但长期使用抗菌药物会导致菌群紊乱,破坏口腔正常生态系统,同时增加细菌对抗菌药物的耐药性,还可引起胃肠道刺激性等不良反应。小檗碱是一种来自药用植物的具有生物活性的天然物质,目前已有成品在临床上使用,其副作用小,能在发挥抗菌作用的同时起到抗炎效果,调节骨组织代谢。小檗碱凭借多途径、多靶点以及多效应的优势在牙周炎治疗中发挥重要作用,其中重要的信号通路包括AMPK、NF-κB和p38MAPK等信号通路。本文就小檗碱在牙周炎治疗中的临床应用及作用机制作一综述,阐述其在牙周炎治疗中的抗菌、抗炎和调节骨代谢的作用,以期为牙周炎治疗提供新思路。Periodontitis is an inflammatory disease that causes chronic,progressive,and irreversible damage to periodontal tissues by a variety of periodontal microbial infections.Antibiotics are currently the first-line drugs for the treatment of periodontitis,such as metronidazole,tinidazole,spiramycin,and other drugs,which can significantly improve periodontal tissue inflammation,but long-term use of antibiotics can cause flora disorders and damage the normal oral ecosystem.At the same time,they increase the resistance of bacteria to antibiotics and can cause adverse reactions such as gastrointestinal irritation.Berberine is a biologically active natural substance derived from medicinal plants.At present,there are finished products for clinical use.It has low incidence of side effects and anti-inflammatory effects,and it can regulate the bone tissue metabolism.Berberine plays an important role in the treatment of periodontitis by its advantages of multiple pathways,multiple targets,and multiple effects.Important signaling pathways include AMPK,NF-κB,p38MAPK,and other signaling pathways.This article summarizes the clinical and research application and mechanism of berberine in the treatment of periodontitis,expounds its antibacterial,anti-inflammatory,and bone metabolism effects in the treatment of periodontitis,and hopes to provide new information for the treatment of periodontitis.
