耳穴按压对维持血液透析患者皮肤瘙痒和IL-31水平的影响

摘　　要：目的:观察耳穴按压对维持血液透析患者皮肤瘙痒症状、白细胞介素-31(interleukin-31,IL-31)水平的影响,并分析IL-31水平和皮肤瘙痒症状的相关性。方法:选取30例血液透析患者给予王不留行籽耳贴耳穴自行按压,每日更换一次耳穴贴,每次穴位按压6~10次,每次至少按压1 min。共贴王不留行籽耳贴4周。比较耳穴按压治疗前后患者的瘙痒视觉模拟量表(visual analogue scale,VAS)评分、5-D瘙痒症状评分、中医证候量表评分和血清IL-31水平,并进一步分析IL-31水平与VAS评分、5-D瘙痒症状评分、中医证候量表评分的相关性。结果:与耳穴按压治疗前比较,耳穴按压后患者的VAS评分、5-D瘙痒症状评分、中医证候量表评分均显著降低(P<0.05),血清IL-31水平显著降低(P<0.05)。上述指标进行相关性分析:IL-31水平分别与VAS评分、5-D瘙痒症状评分、中医证候量表评分呈显著正相关(P<0.05)。结论:耳穴按压可改善血液透析患者皮肤瘙痒症状,降低血清IL-31水平,且瘙痒症状(中医证候)评分和IL-31水平变化呈正相关;耳穴按压改善维持血液透析患者皮肤瘙痒可能与降低血清IL-31水平有关。Objective:To observe the effect of auricular acupoint sticking on itching symptoms and the level of interleukin-31(IL-31)in patients with hemodialysis maintenance,and to analyze the correlation between the level of IL-31 and skin itching symptoms.Methods:Thirty hemodialysis patients were selected and given Wangbuliuxing seeds ear-sticking to make self-press on auricular acupoints.The auricular point sticks were replaced once a day and the acupoints were pressed 6-10 times each time for at least 1 min each time.A total of 4 weeks of Wangbuxing seeds ear acupoint pressing was applied.The pruritus visual analogue scale(VAS)score,5-D pruritus score,TCM syndrome scale score and serum IL-31 level were compared before and after auricular acupuncture treatment,and the IL-31 level and VAS score were further analyzed.,5-D pruritus score,TCM syndrome scale score correlation.Results:Compared with those before auricular acupoint pressing,the VAS score,5-D pruritus symptom score and TCM syndrome scale score of patients after auricular acupoint pressing were significantly decreased(P<0.05),and the serum IL-31 level was significantly decreased(P<0.05).The correlation analysis of the above indicators showed that the level of IL-31 was significantly positively correlated with the VAS score,5-D pruritus score,and TCM syndrome scale score(P<0.05).Conclusion:Auricular acupoint pressing can improve skin itching symptoms in hemodialysis patients,reduce serum IL-31 level,and the itching symptom(TCM syndrome)score is positively correlated with IL-31 level changes of the serum IL-31 level.
