含糠酸莫米松的明胶海绵嗅裂区微填塞治疗嗅觉障碍的近期疗效观察

摘　　要：目的观察含糠酸莫米松的明胶海绵嗅裂区微填塞治疗嗅觉障碍的近期疗效。方法选择病毒感染后嗅觉减退患者74例,49例治疗组采取经鼻内镜下含糠酸莫米松的明胶海绵嗅裂区微填塞进行治疗,25例观察组采取观察自愈情况,所有治疗持续3月,分别在治疗前,治疗后1月、2月、3月对患者双侧鼻腔进行T&T嗅觉计测试嗅觉识别阈,评估患者嗅觉功能和分级,等级资料以秩均值表示。结果治疗组的治疗前、治疗后1月、2月、3月之间的差异有统计学意义(H=55.402,P<0.001),观察组的观察前、观察1月、2月、3月之间的差异无统计学意义(H=1.265,P>0.001),观察1月与治疗后1月之间的差异无统计学意义(H=2.916,P>0.001),观察2月与治疗后2月之间的差异有统计学意义(H=12.840,P<0.001),观察3月与治疗后3月之间的差异有统计学意义(H=20.374,P<0.001)。结论针对病毒感染后嗅觉障碍患者,采取鼻内镜下含糠酸莫米松的明胶海绵嗅裂区微填塞治疗可能一种稳定可靠治疗手段。Objective To survey the short-term efficacy of microtamponade in the olfactory cleft of gelatin sponge containing mometasone furoate on smell disorders.Methods Choose 74 cases of PVOD patients,49 cases of which were slightly stuffed gelatin sponge containing mometasone furoate in their olfactory cleft,25 cases of which were watched the self-healing of their smell,all treatments lasted for 3 months,the olfactory function and grade of all patients were evaluated in pre-treatment,1 month,2 months,3 months of post-treatment with bilateral nasal T&T olfactometer tests olfactory recognition threshold,the rank datas were expressed as rank mean.Results The difference of rank mean of patients in the treatment group was statistically significant(H=55.402,P<0.001)in pre-treatment,1 month,2 months,3 months of post-treatment,the difference of rank mean of patients in the observation group wasn't statistically significant(H=1.265,P>0.001)in pre-treatment,1 month,2 months,3 months of post-treatment,the difference betweenboth groups wasn't statistically significant(H=2.916,P>0.001)1 month later,the difference between both groups was statistically significant(H=12.840,P<0.001)2 months later,the difference between both groupswas statistically significant(H=20.374,P<0.001)3 months later.Conclusion Microtamponade in the olfactory cleft of gelatin sponge containing mometasone furoate by nasal endoscope could be a stable and reliable treatment method for the PVOD patients.
