锁定钢板内固定治疗复杂性胫骨平台骨折的效果研究

摘　　要：目的探讨锁定钢板内固定治疗复杂性胫骨平台骨折的临床效果。方法选取2019年6月—2020年6月该院收治的50例复杂性胫骨平台骨折患者为研究对象,随机分为两组,各25例。对照组采用传统钢板固定治疗,观察组采用锁定钢板内固定治疗,对比两组疗效。结果观察组膝关节功能恢复总有效率为92.00%,高于对照组的68.00%,差异有统计学意义(χ^(2)=4.500,P=0.033)。两组患者手术时间、术中出血量、切口长度对比差异无统计学意义(P>0.05);观察组骨折愈合时间、完全负重时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。术后6、12个月,观察组胫骨平台后倾角(PA)及膝关节活动度均高于对照组,TPA均明显低于对照组,差异有统计学意义(P<0.05)。两组并发症发生率对比差异无统计学意义(P>0.05)。结论在复杂性胫骨平台骨折患者的治疗中锁定钢板内固定术效果理想,具有推广价值。Objective To investigate the clinical effect of locking plate internal fixation in the treatment of complex tibial plateau fractures.Methods 50 patients with complex tibial plateau fractures treated in the hospital from June 2019 to June 2020 were selected and randomly divided into two groups,25 cases in each group.The control group was treated with traditional steel plate fixation,and the observation group was treated with locking steel plate internal fixation.The curative effects of the two groups were compared.Results The total effective rate of knee joint function recovery in the observation group was 92.00%,which was higher than 68.00%in the control group,and the difference was statistically significant(χ^(2)=4.500,P=0.033).There was no statistically significant difference in operation time,intraoperative bleeding and incision length between the two groups(P>0.05).The fracture healing time,complete weight-bearing time and hospital stay in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.05).At 6 months and 12 months after operation,the tibial plateau caster(PA)and knee joint mobility in the observation group were higher than those in the control group,and TPA was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Locking plate internal fixation is effective in the treatment of patients with complex tibial plateau fractures,and it has a promotion value.
