摘 要:目的研究ELISA单试剂抗-HCV反应性献血者的HCV基因分型及归队。方法选取该站2019年1月—2020年12月301名ELISA单试剂检测抗-HCV反应性献血者为研究对象,采集静脉血做血清学及PCR定性检测。PCR定性检测阳性者做HCV基因分型,阴性者做献血者归队处理。结果301名通过ELISA筛查抗-HCV献血者,男性180名,138名HCV NAT无反应,占比达到76.67%,高于女性的61.16%(74/121);HCV NAT反应占比男性为10.56%,小于女性的34.71%;HCV分型阳性数,男性为6.11%,小于女性的14.05%。PCR反应性献血者HCV基因分型阳性者HCV亚型分布分别为:HCV2a占25.00%,HCV3a占17.86%,HCV6a占14.28%,HCV1b占42.86%,结果显示:分布情况为HCV1b>HCV2a>HCV3a>HCV6a。PCR无反应性献血者6个月后再次接受血清学和NAT检测,212名中有182名再次接受检测,其中2种方法均合格的献血者有102名,将其纳入次轮归队程序。经过第二轮的归队程序后,有81名献血者回归献血者队伍,归队为26.91%(81/301)。结论临床中ELISA单试剂抗-HCV反应性献血者的HCV基因分型及归队研究具有重要意义,可筛选出部分HCV阳性者并进行屏蔽,提升献血安全性,值得广泛推广应用。Objective To study the HCV genotyping and return of ELISA single-reagent anti-HCV reactive blood donors.Methods A total of 301 blood donors with anti-HCV reactivity detected by ELISA single-reagent in the station from January 2019 to December 2020 were selected as the research objects.Venous blood was collected for serological and PCR qualitative detection.Those who were positive in PCR qualitative test will be subjected to HCV genotyping,and those who were negative will be treated as blood donors.Results Among 301 anti-HCV blood donors screened by ELISA,180 were male and 138 had no response to HCV NAT,accounting for 76.67%,which was higher than 61.16%(74/121)of females.HCV NAT reaction accounted for 10.56%of males,which was less than 34.71%of females.The number of positive HCV typing was 6.11%in males,less than 14.05%in females.The distribution of HCV subtypes in PCR-reactive blood donors with positive HCV genotypes was as follows:HCV2a accounted for 25.00%,HCV3a accounted for 17.86%,HCV6a accounted for 14.28%,and HCV1b accounted for 42.86%.The results showed that the distribution was HCV1b>HCV2a>HCV3a>HCV6a.PCR non-reactive blood donors received serology and NAT testing again 6 months later.Among 212 cases,182 people were tested again.Among them,102 blood donors who were qualified by both methods were included in the second round of return to the team.After the second round of returning to the team,81 blood donors returned to the blood donor team,and the return rate was 26.91%(81/301).Conclusion The study of HCV genotyping and return of blood donors with ELISA single-reagent anti-HCV reaction in clinical practice is of great significance. It can screen out some HCV positive patients and shield them, and improve the safety of blood donation, which is worthy of widespread application.
