肺胸膜良性转移性黏液样平滑肌瘤一例并文献复习

摘　　要：目的探讨肺胸膜良性转移性黏液样平滑肌瘤的病理特征及鉴别诊断。方法选取1例肺胸膜良性转移性黏液样平滑肌瘤患者,采用巨检、镜检、免疫组化进行检查,观察其病理特征。结果巨检时(右侧胸膜)灰白灰红不整形组织一堆,大小14 cm×13 cm×3 cm,切面灰白灰红,质中,可见黏液性物;(右肺)灰红不整形组织,大小7 cm×4 cm×2 cm,切面灰白,质中,局部黏液样变;镜检时右侧胸膜及右肺瘤组织呈束状、编织样排列,细胞核长梭形,两端钝圆,胞浆红染,间质黏液样变性,黏液组织中的瘤细胞呈星网状,胞浆较少,未见明显异型性及凝固性坏死,核分裂像少见;联合免疫组化确诊为肺胸膜良性转移性黏液样平滑肌瘤。结论肺胸膜良性转移性黏液样平滑肌瘤临床病理特征显著,采用巨检、镜检、免疫组化可进行鉴别诊断。Objective To investigate the pathological features and differential diagnosis of benign metastatic myxoid leiomyoma of the pulmonary pleura.Methods A patient with benign metastatic myxoid leiomyoma of the pulmonary pleura was selected and examined by macroscopic examination,microscopic examination and immunohistochemistry to observe its pathological characteristics.Results At the time of macroscopic examination(right pleura),there was a pile of gray-white gray-red non-plastic tissue,the size was 14 cm×13 cm×3 cm,the cut surface was gray-white gray-red,and mucus was visible in the quality.(Right lung)Gray-red non-plastic tissue,size 7 cm×4 cm×2 cm,gray-white cut surface,medium quality,local myxoid change.On the microscopic examination,the right pleura and the right lung tumor tissue were arranged in bundles and braided patterns,the nuclei were long fusiform,the two ends were blunt and the cytoplasm was stained red.Interstitial myxoid degeneration,the tumor cells in the mucous tissue were stellate,less cytoplasm,no obvious atypia and coagulation necrosis,and mitotic figures were rare.Combined immunohistochemistry was diagnosed as benign metastatic myxoid leiomyoma of the pulmonary pleura.Conclusion The clinicopathological features of benign metastatic myxoid leiomyoma in the pulmonary pleura are significant,and macroscopic examination,microscopic examination and immunohistochemistry can be used for differential diagnosis.
