糖尿病下肢血管病变手术中腰-硬联合麻醉的应用效果分析

摘　　要：目的研究腰-硬联合麻醉在糖尿病下肢血管病变手术中的效果。方法选取该院2019年8月—2021年8月期间收治的糖尿病下肢血管病变手术患者94例,随机分为研究组(n=48)和对照组(n=46),研究组采取腰-硬联合麻醉,对照组采取全身麻醉,比较两组患者的不良反应、麻醉情况,视觉模拟评分(VAS)、简易智力状态检查量表(MMSE)情况。结果研究组的不良反应总发生率显著低于对照组,差异有统计学意义(P<0.05);研究组的麻醉起效时间、术后苏醒时间显著短于对照组,差异有统计学意义(P<0.05);研究组术后2、12、24 h的VAS评分与对照组比较,差异无统计学意义(P>0.05);研究组术后24 h的MMSE评分显著高于对照组,差异有统计学意义(P<0.05)。结论将腰-硬联合麻醉应用于糖尿病下肢血管病变手术中,起效时间快,不良反应少,术后能够较早苏醒,麻醉效果显著,不易损伤认知功能,具有较高的安全性。Objective To study the effect of combined spinal-epidural anesthesia in the operation of diabetic lower extremity vascular disease.Methods 94 patients with diabetic lower extremity vascular disease who were treated in the hospital from August 2019 to August 2021 were selected and randomly divided into a study group(n=48)and a control group(n=46).The study group received combined spinal-epidural anesthesia,and the control group received general anesthesia.The adverse reactions,anesthesia,visual analogue scale(VAS)and mini-mental state examination(MMSE)were compared between the two groups.Results The total incidence of adverse reactions in the study group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).The anesthesia onset time and postoperative recovery time of the study group were significantly shorter than those of the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in VAS scores between the study group and the control group at 2 h,12 h,and 24 h after operation(P>0.05).The MMSE score of the study group at 24 h after operation was significantly higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Combined spinal-epidural anesthesia is applied in the operation of diabetic lower extremity vascular disease,with fast onset time and few adverse reactions.It can wake up earlier after surgery,has a significant anesthesia effect,is not easy to damage cognitive function,and has high safety.
