摘 要:目的探讨喉罩全麻诱导期静脉靶控输注右美托咪定联合丙泊酚的应用效果。方法选取2020年9月—2021年9月于该院行喉罩全麻的86例小儿手术患者,按随机数表法分为两组,各43例。对照组麻醉诱导期予以丙泊酚输注,观察组加用右美托咪定输注。比较两组生命体征变化、术后恢复情况、丙泊酚用量、喉罩一次置入成功率及并发症情况。结果观察组诱导后1 min(T2)、喉罩置入时(T3)时平均动脉压(MAP)[(100.34±9.05)、(106.36±9.27)mmHg]、心率(HR)[(70.13±7.05)、(75.56±7.24)次/min]低于对照组,差异有统计学意义(t=5.198、7.384、6.145、4.252,P<0.001);观察组丙泊酚用量为(382.58±31.81)mg,少于对照组,喉罩拔除、自主呼吸恢复及Steward清醒评分≥4分时间为(10.53±2.14)、(6.81±1.12)、(15.33±2.28)min,短于对照组,差异有统计学意义(t=4.713、6.863、9.338、7.334,P<0.001);观察组喉罩一次置入成功率为100.00%,高于对照组,并发症发生率为4.65%,低于对照组,差异有统计学意义(χ^(2)=4.479、4.074,P=0.034、0.044)。结论喉罩全麻诱导期靶控输注右美托咪定联合丙泊酚应用价值高,可维持患者生命体征稳定,减少丙泊酚用量,缩短术后自主呼吸恢复时间,且利于提高喉罩一次置入成功率,降低喉罩相关并发症发生风险。Objective To explore the effect of intravenous target-controlled infusion of dexmedetomidine combined with propofol during induction of general anesthesia of laryngeal mask.Methods A total of 86 pediatric patients who underwent surgery of laryngeal mask with general anesthesia in the hospital from September 2020 to September 2021 were selected and divided into two groups by random number table method.Each group had 43 cases.The control group was treated with propofol infusion during the induction of anesthesia,and the observation group was given dexmedetomidine infusion combined with propofol during the induction of anesthesia.The changes in vital signs,postoperative recovery,propofol dosage,success rate of laryngeal mask implantation by one time and complications were compared between the two groups.Results Mean arterial pressure(MAP)[(100.34±9.05)mmHg,(106.36±9.27)mmHg],heart rate(HR)[(70.13±7.05)1 min after induction(T2)and laryngeal mask placement(T3)in observation group)times/min,(75.56±7.24)times/min]were lower than those in the control group,and the difference was statistically significant(t=5.198,7.384,6.145,4.252,P<0.001);the dosage of propofol in the observation group was(382.58±31.81)mg,less than the control group,and the time of laryngeal mask removal,spontaneous breathing recovery and Steward wakefulness score≥4 points was(10.53±2.14)min,(6.81±1.12)min,(15.33±2.28)min,shorter than the control group,the difference was statistically significant(t=4.713,6.863,9.338,7.334,P<0.001);the one-time success rate of laryngeal mask insertion in the observation group was 100%,higher than that in the control group,and the complication rate was 4.65%,lower than In the control group,the difference was statistically significant(χ^(2)=4.479,4.074,P=0.034,0.044).Conclusion The target-controlled infusion of dexmedetomidine combined with propofol during the induction of laryngeal mask general anesthesia is of high application value.It can maintain the patients’vital signs,reduce the dosage of propofol,shorte
