前置胎盘治疗中改良式剖宫产术的应用效果及对术中出血量的影响分析

摘　　要：目的探究前置胎盘治疗中改良式剖宫产术的应用效果。方法研究对象为2016年10月—2021年10月于该院妇产科行剖宫产治疗的前置胎盘患者78例,依照电脑随机分组法分为甲、乙两组。甲组39例(传统剖宫产术),乙组39例(改良式剖宫产术),对比不同手术方法对患者影响。结果乙组手术时间(23.69±8.35)min、术中出血量(216.59±25.43)mL及手术-胎儿娩出时间(8.75±1.64)min均优于甲组(41.78±8.93)min、(257.12±26.38)mL、(16.83±2.74)min,差异有统计学意义(t=9.241、6.908、15.802,P<0.05);乙组首次下床活动时间、首次肛门排气时间、住院时间、恶露持续时间及月经复潮时间均明显短于甲组,差异有统计学意义(P<0.05),且乙组术后VAS评分明显低于甲组,差异有统计学意义(P<0.05);乙组术后并发症发生率5.13%明显低于甲组20.51%,差异有统计学意义(χ^(2)=4.129,P<0.05);乙组患者对手术满意度(94.87%)明显高于甲组(79.49%),差异有统计学意义(χ^(2)=4.129,P<0.05)。结论对于前置胎盘患者,选择改良式剖宫产术治疗效果要优于传统剖宫产术,可以预防术后并发症发生,减少术中出血量,缩短住院时间,手术效果为人们所接收认可。Objective To explore the effect of modified cesarean in placenta previa treatment.Methods The study subjects were 78 patients with placenta previa who underwent cesarean section in the department of Obstetrics and Gynecology of the hospital from October 2016 to October 2021.They were divided into two groups according to the computer random grouping method,with 39 cases in group A(traditional cesarean)and 39 cases in group B(modified cesarean).The effects of different surgical methods on patients were compared.Results The operation time(23.69±8.35)min,intraoperative blood loss(216.59±25.43)mL and operative-fetal delivery time(8.75±1.64)min in group B were better than those in group A(41.78±8.93)min,(257.12±26.38)mL,(16.83±2.74)min,the difference was statistically significant(t=9.241,6.908,15.802,P<0.05).The time of first ambulation,first anal exhaust,hospital stay,duration of lochia and period of menstruation in group B were significantly shorter than those in group A,the difference was statistically significant(P<0.05).And the postoperative VAS score in group B was significantly lower than that in group A,the difference was statistically significant(P<0.05).The incidence of postoperative complications in group B(5.13%)was significantly lower than that in group A(20.51%),the difference was statistically significant(χ^(2)=4.129,P<0.05).Surgical satisfaction in group B(94.87%)was significantly higher than that in group A(79.49%),the difference was statistically significant(χ^(2)=4.129,P<0.05).Conclusion For patients with placenta previa,the treatment effect of modified cesarean is better than traditional cesarean.Modified cesarean can prevent postoperative complications,reduce intraoperative blood loss,shorten the hospital stay,and the surgical effect is accepted by people.
