诺舒子宫内膜去除术及宫腔镜子宫内膜电切方案用于子宫异常出血治疗的临床价值

摘　　要：目的对比诺舒子宫内膜去除术和宫腔镜子宫内膜电切术对子宫异常出血的治疗效果。方法随机选取2016年10月—2020年12月就诊于石河子大学医学院第一附属医院的子宫异常出血患者80例进行研究,根据患者个人治疗意愿分成对照组和观察组。对照组采用宫腔镜子宫内膜电切术,观察组予以诺舒子宫内膜去除术,对比两组临床疗效、术中指标、术后指标和患者满意度。结果观察组总有效率为95.00%,满意度为97.50%,高于对照组的77.50%和80.00%,差异有统计学意义(χ^(2)=5.164、4.507,P<0.05);两组患者出现子宫穿孔、电解质紊乱的概率和PBAC评分差异无统计学意义(P>0.05);观察组手术时间,发热、腹痛、阴道排液持续时间更短,手术出血量更少,与对照组比较差异有统计学意义(P<0.05)。结论与宫腔镜子宫内膜电切术相比,予以子宫异常出血患者诺舒子宫内膜去除术,效果明显,可缩短手术时间,减少出血量,利于患者术后恢复,使得患者满意度提高,存在推广价值。Objective To compare the therapeutic effect of Nuoshu endometrial ablation and hysteroscopic endometrial resection on abnormal uterine bleeding.Methods A total of 80 patients with abnormal uterine bleeding who were treated in the First Affiliated Hospital of Shihezi University School of Medicine from October 2016 to December 2020 were random selected for the study.The patients were divided into the control group and the observation group according to their individual treatment willingness.The control group was treated with hysteroscopic endometrial resection,and the observation group was treated with Nuoshu endometrial ablation.The clinical efficacy,intraoperative indicators,postoperative indicators and patient satisfaction were compared between the two groups.Results The total effective rate in the observation group was 95.00%,and the satisfaction rate was 97.50%,which was higher than 77.50%and 80.00%in the control group,and the difference was statistically significant(χ^(2)=5.164,4.507,P<0.05).There was no statistically significant difference in the probability of electrolyte disturbance and PBAC score(P>0.05);compared with the control group,the observation group had shorter operation time,shorter duration of fever,abdominal pain,and vaginal discharge,and less operative blood loss,and the differences were statistically significant(P<0.05).Conclusion Compared with hysteroscopic endometrial resection,Nuoshu endometrial ablation in patients with abnormal uterine bleeding has obvious effect,which can shorten the operation time,reduce the amount of bleeding,and facilitate the postoperative recovery of patients,which improves patient satisfaction and has promotion value.
