摘 要:目的探究西格列汀对应用大剂量胰岛素效果欠佳的2型糖尿病的疗效。方法方便选取2019年2月—2021年6月该院收治的82例大剂量胰岛素效果欠佳的2型糖尿病患者作为研究对象,并以随机抽样法为分组依据,分为研究组(西格列汀)与对照组(二甲双胍),各41例,对干预效果进行分析比较。结果研究组总有效率(97.56%)较对照组(78.05%)高,差异有统计学意义(χ^(2)=7.289,P<0.05),研究组不良反应发生率(9.76%)与对照组(14.63%)比较,差异无统计学意义(χ^(2)=0.456,P>0.05),用药前,两组餐后血糖(FBG)、餐后2 h血糖(2 hBG)、糖化血红蛋白(HbA1c)及每日胰岛素剂量指标比较,差异无统计学意义(t=0.044、0.079、0.620、0.204,P>0.05),用药后,上述指标优于用药前,且研究组用药后均较对照组低,差异有统计学意义(t=14.227、7.574、11.256、19.402,P<0.05),用药前,两组HOMA-β、HOMA-IR指标比较,差异无统计学意义(t=0.106、0.085,P>0.05),用药后,上述指标优于用药前,研究组用药后HOMA-β高于对照组,HOMA-IR低于对照组,差异有统计学意义(t=13.552、2.458,P<0.05)。结论西格列汀可控制患者血糖水平,减少每日胰岛素剂量,改善胰岛功能,值得应用。Objective To investigate the effect of sitagliptin on type 2 diabetes mellitus with poor response to high-dose insulin.Methods From February 2019 to June 2021,82 patients with type 2 diabetes who had not well received high-dose insulin admitted in the hospital were conveniently selected as the research subjects.According to the random sampling method,they were divided into study group(sitagliptin)and control group(metformin),with 41 cases in each group.The intervention effects were analyzed and compared.Results The total effective rate of the study group was 97.56%higher than that of the control group(78.05%),the difference was statistically significant(χ^(2)=7.289,P<0.05).The incidence of adverse reactions in the study group was 9.76%compared with 14.63%in the control group,and the difference was not statistically significant(χ^(2)=0.456,P>0.05),before medication,there was no statistically significant difference in postprandial blood glucose(FBG),2 h postprandial blood glucose(2 hBG),glycated hemoglobin(HbA1c)and daily insulin dose between the two groups(t=0.044,0.079,0.620,0.204,P>0.05),after medication,the above indexes were better than before treatment,and the study group was lower than the control group after treatment,the difference was statistically significant(t=14.227,7.574,11.256,19.402,P<0.05),before medication,the indexes of HOMA-βand HOMA-IR in the two groups were compared,the difference was not statistically significant(t=0.106,0.085,P>0.05),after medication,the above indexes were better than those before medication,and HOMA-βafter medication in the study group higher than the control group,HOMA-IR lower than the control group,the difference was statistically significant(t=13.552,2.458,P<0.05).Conclusion Sitagliptin can control the blood glucose level,reduce the daily insulin dose,and improve the islet function,and it is worthy of application.
