超声心动图对12~15^(+6)周肥胖孕妇胎儿应用价值的研究

摘　　要：目的研究肥胖孕妇有无必要增加12~15^(+6)周的中孕早期胎儿超声心动图检查。方法收集接受产前超声检查的孕12~15^(+6)周孕妇共453例,按孕妇孕前身体重指数(BMI)将其分为肥胖组156例,正常组297例。采集2组胎儿心脏四腔心切面、三血管气管切面以及左、右室流出道切面,随访每位孕妇至20~24+6周的中孕期再次采集胎儿心脏上述4个切面,统计分析12~15^(+6)周的中孕早期、20~24+6周的中孕期以及两孕期联合检查时2组孕妇胎儿超声心动图检查的完成率。结果肥胖组孕妇孕前BMI较正常组高,差异有统计学意义(P<0.05),2组间孕妇年龄及孕周差异均无统计学意义(P均>0.05),肥胖组孕妇中孕期胎儿超声心动图检查完成率较正常组低,差异有统计学意义(P<0.01);肥胖组孕妇中孕早期胎儿超声心动图检查完成率较中孕期高,但差异无统计学意义(P>0.05);肥胖组孕妇两孕期联合胎儿超声心动图检查完成率较中孕早期高,差异有统计学意义(P<0.01);肥胖组孕妇两孕期联合胎儿超声心动图检查完成率较中孕期高,差异有统计学意义(P<0.01);正常组孕妇中孕期胎儿超声心动图检查完成率较中孕早期高,但差异无统计学意义(P>0.05);正常组孕妇两孕期联合胎儿超声心动图检查完成率较中孕早期高,差异有统计学意义(P<0.01);正常组孕妇两孕期联合胎儿超声心动图检查完成率较中孕期高,但差异无统计学意义(P>0.05)。结论肥胖孕妇,增加12~15^(+6)周的中孕早期胎儿超声心动图检查可以更好的了解胎儿心脏结构的发育情况。Objective To investigate the application value of fetal echocardiography in the obesity pregnant women at first trimester of pregnancy from 12 weeks to 15^(+6) weeks.Methods A total of 453 pregnant women with pregnant week being 12 to 15^(+6) weeks who received ultrasound examination in our hospital were enrolled in the study,who were divided into the obese group(n=156)and the normal group(n=297)according to body mass index(BMI)before pregnancy.The four chamber view,three vessel and trachea view,as well as left and right ventricular outflow tract view were collected,and the pregnant women were followed up to the second trimester,and above four fetal cardiac views were collected.In addition,the completion rates of fetal echocardiography during the early second trimester of 12 to 15^(+6) weeks,the second trimester of 20 to 24+6 weeks,and the combined examination of the two gestational periods were statistically analyzed.Results Before pregnancy,the BMI levels in obese group were significantly higher than those in normal group(P<0.05),however,there were no significant differences in maternal age and gestational age between the two groups(P>0.05).The completion rate of fetal echocardiography in obese group was significantly lower than that in normal group(P<0.01).The completion rate of fetal echocardiography in the early second trimester was higher than that in the second trimester,but there was no significant difference(P>0.05).The completion rate of combined examination in obese group was significantly higher than that in the early second trimester and that in the second trimester(P<0.01).Moreover the completion rate of fetal echocardiography in the early second trimester was lower than that in the second trimester,but there was no significant difference(P>0.05).The completion rate of combined examination in normal group was significantly higher than that in the early second trimester(P<0.01),which was higher than that in the second trimester,however,the difference was not statistically significant(P>0.05).Conclusi
