参附注射液对于脓毒症肺损伤机械通气患者炎症因子水平及凝血功能的影响

摘　　要：目的探究参附注射液治疗脓毒症肺损伤机械通气患者的效果,观察炎症因子水平及凝血功能的变化。方法随机选取2018年3月—2021年5月收治的脓毒症肺损伤机械通气患者50例,采用随机数表法将其分为对照组和观察组,每组25例。对照组给予标准抗感染、呼吸支持等措施,观察组在常规措施的基础上联合参附注射液治疗。比较两组患者治疗前后炎症因子水平及凝血功能变化。结果与治疗前相比,治疗后,观察组患者白细胞(9.57±2.40)×10^(9)/L、C反应蛋白(101.30±17.00)mg/L、白细胞介素6(28.68±6.33)ng/L、肿瘤坏死因子α(42.40±11.36)μg/L水平均降低,与对照组比,观察组C反应蛋白、白细胞介素6、肿瘤坏死因子α指标下降明显,差异有统计学意义(t=4.383、3.741、3.443,P<0.05);通过治疗,观察组患者凝血酶原时间(13.24±2.11)s、部分凝血酶原时间(39.52±4.30)s缩短,D二聚体水平(2659±1042)μg/L也较治疗前下降,且观察组改善水平优于对照组,差异有统计学意义(t=2.496、2.208、2.673,P<0.05)。观察组纤维蛋白原(3.79±0.81)g/L明显高于对照组,差异有统计学意义(t=2.668,P<0.05)。结论参附注射液治疗脓毒症肺损伤机械通气患者可降低炎症水平,改善凝血功能。Objective To explore the effect of Shenfu injection in the treatment of septic lung injury patients with mechanical ventilation,and observe the changes of inflammatory factor levels and blood coagulation function.Methods A total of 50 septic lung injury mechanically ventilated patients who were admitted from March 2018 to May 2021 were selected and divided into a control group and an observation group by a random number table method,with 25 cases in each group.Patients in the control group need to be given standard anti-infection,respiratory support and other measures,and the observation group was treated with Shenfu injection on the basis of conventional measures.The levels of inflammatory factors and changes in blood coagulation function were compared between the two groups of patients before and after treatment.Results Compared with those before treatment,after treatment,the observation group had leukocytes(9.57±2.4)×10^(9)/L,C-reactive protein(101.30±17.00)mg/L,interleukin 6(28.68±6.33)ng/L,tumors necrosis factor alpha(42.40±11.36)μg/L all decreased,compared with the control group,the C-reactive protein,interleukin-6,and tumor necrosis factorαindexes of the observation group decreased significantly,the difference was statistically significant(t=4.383,3.741,3.443,P<0.05).Through treatment,the prothrombin time(13.24±2.11)s and partial prothrombin time(39.52±4.30)s of the observation group were shortened,and the D-dimer level(2659±1042)μg/L was also lower than before treatment,and the improvement level of the observation group was better than that of the control group,the difference was statistically significant(t=2.496,2.208,2.673,P<0.05).The fibrinogen(3.79±0.81)g/L in the observation group was significantly higher than that in the control group,the difference was statistically significant(t=2.668,P<0.05).Conclusion Shenfu injection treated septic lung injury patients with mechanical ventilation can reduce inflammation and improve blood coagulation function.
