足月胎膜早破应用小剂量米索前列醇与缩宫素引产的临床效果

摘　　要：目的分析在足月胎膜早破临床治疗中应用小剂量米索前列醇与缩宫素进行引产的效果。方法研究简单随机选取2019年2月—2021年3月该院进行诊治的足月胎膜早破患者100例,根据单盲分组方式对患者进行分组,参照组(50例)、观察组(50例),参照组以缩宫素为患者进行引产,观察组应用小剂量米索前列醇为患者进行引产,对两组患者引产成功率、用药后的临床指标以及不良事件发生率进行比较。结果观察组引产成功率(86.00%)、不良事件发生率(8.00%),观察组患者用药到临产时间(5.71±1.15)h、用药到分娩时间(10.98±2.01)h、总产程(5.79±1.26)h以及产后失血量(121.84±38.93)mL,均明显优于参照组,差异有统计学意义(χ^(2)=5.482、5.740,P<0.05;t=13.311、16.438、12.329、5.196,P<0.001)。结论足月胎膜早破患者应用小剂量米索前列醇进行引产,可以有效提升引产成功率,控制患者产后出血量,药效更为迅速,对于降低患者分娩不良事件发生率具有积极意义,具有安全性。Objective To analyze the effect of low-dose misoprostol and oxytocin for labor induction in the clinical treatment of term premature rupture of membranes.Methods The study objects were 100 cases with term premature rupture of membranes diagnosed and treated in the hospital were simple randomly selected from February 2019 to March 2021.The patients were divided into two groups according to the single blind grouping method,50 cases in the reference group and 50 cases in the observation group,the reference group used oxytocin as the patient to induce labor,and the observation group used low-dose misoprostol to induce labor.The success rate of induced labor,clinical indexes and incidence of adverse events were compared between the two groups.Results The success rate of labor induction in the observation group(86.00%),the incidence of adverse events(8.00%),the time from medication to labor(5.71±1.15)h,the time from medication to delivery(10.98±2.01)h,and the total labor process(5.79±1.26)h in the observation group and postpartum blood loss(121.84±38.93)mL were significantly better than the reference group,the difference was statistically significant(χ^(2)=5.482,5.740,P<0.05;t=13.311,16.438,12.329,5.196,P<0.001).Conclusion The application of low-dose misoprostol to induce labor in patients with term premature rupture of membranes can effectively increase the success rate of labor induction,control the patient's postpartum hemorrhage,and have a faster drug effect.It is of positive significance and safety for reducing the incidence of adverse childbirth events in patients.
