全功能动态矫正衣辅助治疗小儿痉挛型脑瘫40例临床观察

摘　　要：目的观察全功能动态矫正衣结合针刺及常规运动康复训练对痉挛型脑瘫患儿粗大运动功能的影响。方法选取2017年8月至2020年8月嘉兴市中医医院康复科门诊和住院部收治的痉挛型脑瘫患儿80例,采用随机数字表法分为对照组和治疗组,各40例。对照组予针刺联合常规运动康复训练治疗,治疗组在对照组治疗方法的基础上穿全功能动态矫正衣,2组均每周治疗5 d,2周为1个疗程,治疗6个疗程后统计疗效。结果治疗前2组的粗大运动能力测定(GMFM-66)量表评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组的GMFM-66量表中B、C、D、E 4个区评分均较同组治疗前明显升高,差异均有统计学意义(P<0.01),且治疗组升高更显著(P<0.01)。治疗前2组的Peabody粗大运动量表(PDMS-2)评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组的PDMS-2量表中姿势和移动评分均较同组治疗前明显升高,差异均有统计学意义(P<0.01),且治疗组升高更显著(P<0.01)。结论全功能动态矫正衣辅助治疗痉挛型脑瘫患儿,可显著提高GMFM-66及PDMS-2量表评分,有效改善患儿的姿势保持、行走跑跳等粗大运动功能发育,值得临床推广应用。Objective To observe the effects of full-function dynamic corrective clothing worn by children patients combined with acupuncture and conventional motor rehabilitation activities on gross motor function of children with spastic cerebral palsy(CP).Methods From August 2017 to August 2020,eighty children with spastic CP admitted into outpatient service and inpatient department of rehabilitation department of Jiaxing Hospital of TCM were divided into control group and treatment group according to random number table method,with 40 cases in each group.The control group was given acupuncture combined with conventional motor rehabilitation activities,on this basis,the treatment group wore full-function dynamic corrective clothing.Both groups were treated for 5 days per week,2 weeks as 1 course of treatment,and the curative effect was calculated after 6 courses of treatment.Results Before treatment,there was no statistically significant difference in gross motor function measure-66(GMFM-66)scale scores between the 2 groups(P>0.05),indicating comparability.After treatment,the scores of B,C,D and E 4 dimensions of GMFM-66 scale in the 2 groups were significantly higher than those in the same group before treatment,and the difference was statistically significant(P<0.01),moreover,the increase was more significant in the treatment group(P<0.01).Before treatment,there was no statistically significant difference in Peabody developmental motor scales-second edition(PDMS-2)scores between the 2 groups(P>0.05),indicating comparability.After treatment,the scores of stationary category and locomotion category in PDMS-2 of both groups increased significantly than those in the same group before treatment,and the differences were statistically significant(P<0.01),moreover,the increase was more significant in the treatment group(P<0.01).Conclusion Full-function dynamic corrective clothing,worn by children patients in adjunctive treatment of children with spastic CP,can markedly improve their GMFM-66 and PDMS-2 scales scores and effectiv
