超声多普勒角度对阴茎海绵体动脉血流检测结果的影响

摘　　要：目的观察勃起功能障碍患者行超声检查时多普勒角度变化对阴茎海绵体动脉血流检测结果的影响,探讨最佳检测多普勒角度。方法勃起功能障碍患者41例,均于阴茎注射前列地尔阴茎勃起达最大硬度后行彩色多普勒超声检查,分别测量多普勒角为0°、30°、60°、75°时阴茎海绵体动脉收缩期峰值流速(peak systolic velocity,PSV)及舒张末期流速(end diastolic velocity,EDV)并进行比较。结果左、右侧阴茎海绵体动脉PSV在多普勒角为0°[(31.00±7.33)、(32.85±8.68)cm/s]、30°[(35.79±8.47)、(37.93±10.03)cm/s]、60°[(61.99±14.66)、(65.69±17.36)cm/s]时均低于75°时[(119.68±28.31)、(126.81±33.52)cm/s](P<0.05),0°、30°时均低于60°时(P<0.05),0°时均低于30°时(P<0.05)。左、右侧阴茎海绵体动脉EDV在多普勒角为0°[(-3.82±1.21)、(-4.34±2.40)cm/s]、30°[(-4.41±1.40)、(-5.01±2.77)cm/s]、60°[(-7.63±2.42)、(-8.68±4.80)cm/s]时均高于75°时[(-14.73±4.67)、(-16.76±9.27)cm/s](P<0.05),0°、30°时均高于60°时(P<0.05),0°时均高于30°时(P<0.05)。结论多普勒角为60°且角度校正线与阴茎海绵体动脉走向平行时测得PSV、EDV结果较为准确,多普勒角度改变尤其是增至75°时明显影响检测结果。Objective To observe the influence of different ultrasound Doppler angles on the blood flow of the penile cavernous artery in patients with erectile dysfunction,and to investigate the best angle.Methods Forty-one erectile dysfunction patients received intracavernous injection of alprostadil till the penile erectile got the maximum hardness.Color Doppler ultrasonography was done to measure the peak systolic velocity and end diastolic velocity of the penile cavernous artery at Doppler angle of 0°,30°,60°and 75°.Results The peak systolic velocities of the left and right cavernous artery increased gradually in turn at Doppler angle of 0°[(31.00±7.33),(32.85±8.68)cm/s],30°[(35.79±8.47),(37.93±10.03)cm/s],60°[(61.99±14.66),(65.69±17.36)cm/s]and 75°[(119.68±28.31),(126.81±33.52)cm/s](P<0.05),and the end diastolic velocities of the left and right cavernous artery decreased gradually in turn at Doppler angle of 0°[(-3.82±1.21),(-4.34±2.40)cm/s],30°[(-4.41±1.40),(-5.01±2.77)cm/s],60°[(-7.63±2.42),(-8.68±4.80)cm/s]and 75°[(-14.73±4.67),(-16.76±9.27)cm/s](P<0.05).Conclusions When the Doppler angle is 60°and the angle correction line is parallel to the cavernous artery,the peak systolic and diastolic velocities are more objective and reliable.Any change of the Doppler angle,especially when the Doppler angle gets 75°,may affect the ultrasonography results significantly.
