摘　　要：目的:探讨心电触发非对比增强磁共振下肢静脉成像的临床应用价值。方法:选取30例临床诊断下肢静脉曲张患者,均采用心电触发非对比增强3D TRANCE序列及下肢静脉X线造影进行下肢静脉检查。由2名5年以上MRI诊断经验的放射科医师对两种方法获得下肢静脉血管影像的图像质量进行评分。采用ICC组内相关系数,评价两名医生对3D TRANCE MRV评分一致性。以下肢静脉X线造影作为金标准,分析3D TRANCE MRV诊断下肢静脉曲张的灵敏度、特异性、准确度、阳性预测率、阴性预测率;采用Kappa检验分析两种方法图像质量评分一致性。结果:采用心电触发非对比增强3D TRANCE序列下肢静脉各段图像Likert评分可重复性好,ICC>0.970。与下肢静脉X线造影比较,3D-TRANCE MRV灵敏度、特异性、阳性预测率、阴性预测率和准确率分别为95.00%、40.00%、95.00%、60.00%和91.11%。结论:3D-TRANCE磁共振技术能够清晰显示下肢静脉曲张,图像质量良好,具有较高诊断准确度,可成为评估下肢静脉曲张的替代检查方法。Objective: To evaluate the clinical value of ECG-triggered non-contrast enhanced magnetic resonance venography of lower extremities.Methods:30 patients with varicose veins of lower extremities were examined by ECG triggered non-contrast enhanced 3 DTRANCE sequence and lower limb venography. The image quality of venous images of lower extremities obtained by the two methods was evaluated by two radiologists with more than 5 years’ experience in the diagnosis of MRI. The intra-group correlation coefficient of ICC was used to evaluate the consistency of 3 D TRANCE MRV scores between the two doctors. Lower limb venography was used as the gold standard to analyze the sensitivity, specificity, accuracy, positive predictive rate and negative predictive rate of 3 D TRANCE MRV in the diagnosis of varicose veins of lower extremities, and Kappa test was used to analyze the consistency of image quality scores of the two methods. Results:the Likert score of each segment of limb vein in non-contrast enhanced 3 DTRANCE sequence triggered by inferior ECG was reproducible(ICC>0.970). Compared with lower limb venography, the sensitivity, specificity, positive predictive rate, negative predictive rate and accuracy of 3 D-TRANCEMRV were 95.00%, 40.00%, 95.00%, 60.00% and 91.11%. Conclusion:3 D-TRANCE magnetic resonance imaging can clearly display varicose veins of lower extremities with good image quality and high diagnostic accuracy, so it can be used as an alternative method for evaluating varicose veins of lower extremities.
