申脉穴结合升陷汤治疗大气下陷证三例

摘　　要：文章以申脉穴结合升陷汤治疗咳嗽、带状疱疹、失眠三病例为切入点,介绍申脉穴结合升陷汤治疗大气下陷的临床要点。大气下陷证为张锡纯在《医学衷中参西录》中首次提出,大气即为心肺之气,大气下陷可出现头晕头疼、胸闷气短、心慌心悸等一系列症状。申脉穴具有补阳益气、疏导水湿之功效。升陷汤由黄芪、知母、桔梗、柴胡、升麻组成,具有补气升提之功效。文章三病例虽症状各异,然病机与脉象相同,脉象皆为沉迟微弱,关前尤甚,病机则为大气下陷证,因此治以申脉穴结合升陷汤取效甚捷。查未见申脉穴结合升陷汤治疗大气下陷证相关报道,特记以丰富本证的治疗。This paper takes acupuncture at Shenmai point(申脉穴)plus the Shengxian decoction(升陷汤)in the treatment of cough,herpes zoster and insomnia as the breakthrough points,and introduces the clinical points of acupuncture at Shenmai point plus the Shengxian decoction in the treatment of Daqi subsidence syndrome(大气下陷证).Atmospheric subsidence syndrome was first proposed by ZHANG Xichun in Records of traditional Chinese and western medicine in combination(《医学衷中参西录》),Daqi is the Qi(气)of the heart and lungs,and subsidence of Daqi can cause a series of symptoms such as dizziness,headache,chest tightness,shortness of breath,palpitation and palpitations.Acupuncture at Shenmai point has the effect of tonifying Yang(阳),nourishing Qi,and dredging water and dampness.The Shengxian decoction is composed of Huangqi(Radix Astragali seu Hedysari),Zhimu(Rhizoma Anemarrhenae),Jiegeng(Radix Platycodonis),Chaihu(Radix Bupleuri),Shengmai(Rhizoma Cimicifugae),which has the effect of tonifying Qi and lifting.Although the symptoms of the three cases in this article are different,the pathogenesis is the same as the pulse pattern.The pulse pattern is slow and weak,and the pathogenesis is Daqi subsidence syndrome.Therefore,the combination of acupuncture at Shenmai point and the Shengxian decoction is very effective.There is no report on acupuncture at Shenmai point plus the Shengxian decoction in the treatment of Daqi subsidence syndrome,and it is specially noted to enrich the treatment of this syndrome.
