免煎颗粒剂痛风关节膏外敷治疗急性痛风性关节炎临床疗效观察

摘　　要：目的:观察免煎颗粒剂痛风关节膏外敷治疗急性痛风性关节炎的临床疗效。方法:选择90例辨证分型为湿热蕴结型的急性痛风患者,按随机数字表法分为治疗组和对照1组、对照2组。治疗组采用免煎颗粒剂痛风关节膏联合秋水仙碱治疗,对照1组采用饮片剂痛风关节膏联合秋水仙碱治疗,对照2组口服秋水仙碱片,观察其临床疗效。结果:治疗组总有效率为93.3%(28/30),对照1组总有效率为86.7%(26/30),对照2组总有效率为63.3%(19/30),治疗组、对照1组分别与对照2组比较,差异有统计学意义(P<0.05),表明内服秋水仙碱联合外敷痛风关节膏临床疗效明显优于单纯口服秋水仙碱。三组患者治疗前后中医证候评分、生化指标比较,差异具有统计学意义,P<0.05,表明三组疗法对急性痛风性关节炎均有疗效,治疗后治疗组和对照1组优于对照2组,P<0.05。治疗组疼痛程度、疼痛发作频率显著低于对照1组和对照2组,P<0.01,表明治疗组在缓解关节疼痛方面效果显著。结论:内服秋水仙碱联合外敷痛风关节膏临床疗效明显优于单纯口服秋水仙碱,免煎颗粒剂与饮片剂痛风关节膏两者的临床有效率无显著差异,且免煎颗粒剂痛风关节膏缓解疼痛的效果显著,优于饮片剂。Objective:To observe the clinical efficacy of gout joint poultice decoction-free granules for external application on acute gouty arthritis.Methods:90 patients were randomly divided into three groups:the treatment group,the control group 1 and the control group 2.The treatment group was treated with gout joint poultice decoction-free granules for external application plus colchicine.The control group 1 was treated with gout joint poultice decoction-free granules for external application plus colchicine.The control group 2 was treated with colchicine tablets.The clinical effect was observed.Results:The total effective rate in the treatment group was 93.3%(28/30),in the control group 1 was 86.7%(26/30),and in the control group 2 was 63.3%(19/30).The treatment group and the control group 1were compared with the control group 2,and the difference was statistically significant(P<0.05).It shows that the clinical effect of oral colchicine plus gout joint poultice decoction-free granules for external application is significantly better than oral colchicine alone.Before and after treatment,TCM symptom scores and biochemical indexes in the three groups were compared,the difference was statistically significant(P<0.05).The results showed that the three groups gained curative effect on acute gouty arthritis.Comparison between groups after treatment,the treatment group and the control group 1 were better than the control group 2(P<0.05).The degree and frequency of pain in treatment group was significantly lower than the control group 1 and the control group 2(P<0.01).It shows that the treatment group has significant effect in relieving joint pain.Conclusion:The clinical efficacy of oral colchicine plus gout joint poultice decoction-free granules for external application was significantly better than oral colchicine alone.There was no significant difference in the clinical efficiency between the treatment group and the control group 1,the pain relief effect of gout joint poultice decoction-free granules for external applicatio
