醒脑针刺疗法结合正中神经电刺激对创伤性颅脑损伤后昏迷患者的影响

摘　　要：目的探讨醒脑针刺疗法结合正中神经电刺激(MNS)对创伤性颅脑损伤后昏迷患者的治疗效果。方法随机选取2018年1月—2020年12月入组的昏迷患者60例为研究对象,以随机分配法将患者分为治疗组和对照组,各30例,两组均给予常规促醒康复治疗,共4周。治疗组在常规治疗的基础上加用MNS治疗及醒脑针刺疗法,比较治疗前和治疗2、4周时格拉斯哥昏迷评分(GCS)、脑功能障碍评分(DRS)、脑电图(EEG)和听觉脑干诱发电位(ABR)评分。结果治疗4周后,治疗组的GCS、EEG和ABR评分(11.97±1.75)分、(9.80±1.30)分和(3.90±0.31)分均高于对照组的GCS、EEG和ABR评分(9.77±1.22)分、(7.53±1.63)分和(2.77±0.68)分,差异有统计学意义(t=-5.641、-5.950、-8.340,P<0.05);治疗4周后,治疗组的DRS评分(5.27±1.36)分明显低于对照组评分(8.60±1.59)分,差异有统计学意义(t=8.722,P<0.05)。结论采用醒脑针刺疗法结合正中神经电刺激的治疗方案对患者促醒的作用更为显著。Objective To explore the therapeutic effect of Xingnao acupuncture combined with median nerve stimulation(MNS)on comatose patients after traumatic brain injury.Methods 60 coma patients who were enrolled in the group from January 2018 to December 2020 were randomly selected as the research objects,and the patients were divided into treatment group and control group by random assignment method,with 30 cases in each group,and both groups were given routine wake-up promoting rehabilitation treatment.The treatment group received MNS therapy and Xingnao acupuncture therapy on the basis of conventional treatment,and the glasgow coma scale(GCS),brain dysfunction score(DRS),electroencephalogram(EEG)and auditory brainstem evoked potentials(ABR)scores were compared before treatment and at 2 and 4 weeks of treatment.Results After four weeks of treatment,the GCS,EEG and ABR scores of the treatment group(11.97±1.75)points,(9.80±1.30)points and(3.90±0.31)points were higher than those of the control group(9.77±1.22)points,(7.53±1.63)points and(2.77±0.68)points,the differences were statistically significant(t=-5.641,-5.950,-8.340,P<0.05).After four weeks of treatment,the DRS score of the treatment group(5.27±1.36)points was significantly lower than that of the control group(8.60±1.59)points,and the difference was statistically significant(t=8.722,P<0.05).Conclusion The use of Xingnao acupuncture combined with median nerve electrical stimulation has a more significant effect on patients'awakening,which is better than that of the control group.
