足踝创伤性关节炎应用踝关节融合术治疗的临床疗效

摘　　要：目的评估踝关节融合术治疗足踝创伤性关节炎的临床效果。方法方便选取2017年10月—2021年3月该院收治的94例足踝创伤性关节炎患者为研究对象,随机均分为两组,各47例。对照组予保守治疗,观察组则予踝关节融合术。对比两组治疗前后踝后足评分、临床效果及生活质量。结果治疗后,观察组疼痛(40.32±2.64)分、功能(38.01±2.85)分、X线(8.15±1.02)分、踝后足评分总分(80.78±3.46)分,高于对照组(24.38±2.55)分、(21.26±2.59)分、(4.12±0.82)分、(61.29±3.41)分,差异有统计学意义(t=-29.773、-29.818、-21.111、-27.505,P<0.05)。观察组临床有效率(93.62%)高于对照组(76.60%),差异有统计学意义(χ^(2)=5.371,P=0.020)。观察组生理(22.01±2.42)分、肢体(21.34±2.32)分、社会(21.17±2.66)分、精神(22.15±2.78)分及总评分(90.35±3.86)分,高于对照组(16.02±1.92)分、(15.87±1.99)分、(16.05±2.13)分、(16.62±2.11)分、(71.51±3.03),差异有统计学意义(t=-13.293、-12.269、-10.300、-10.863、-26.321,P<0.05)。结论对足踝创伤性关节炎运用踝关节融合术效果理想。Objective To evaluate the clinical effect of ankle arthrodesis in the treatment of traumatic arthritis of the foot and ankle.Methods 94 patients with traumatic arthritis of foot and ankle treated in the hospital from October 2017 to March 2021 were conveniently selected and randomly divided into two groups,47 cases in each group.The control group was treated conservatively;the observation group was treated with ankle fusion.The ankle and hindfoot scores,clinical effects and quality of life were compared between the two groups before and after treatment.Results After treatment the total scores of pain(40.32±2.64)points,function(38.01±2.85)points,X-ray(8.15±1.02)points and ankle hind foot(80.78±3.46)points in the observation group were higher than those in the control group(24.38±2.55)points,(21.26±2.59)points,(4.12±0.82)points and(61.29±3.41)points,the difference was statistically significant(t=-29.773,-29.818,-21.111,-27.505,P<0.05).The clinical effective rate of the observation group(93.62%)was higher than that of the control group(76.60%),and the difference was statistically significant(χ^(2)=5.371,P=0.020).The scores of physiology(22.01±2.42)points,limbs(21.34±2.32)points,society(21.17±2.66)points,spirit(22.15±2.78)points and total score(90.35±3.86)points in the observation group were higher than those in the control group(16.02±1.92)points,(15.87±1.99)points,(16.05±2.13)points,(16.62±2.11)points,(71.51±3.03)points,the difference was statistically significant(t=-13.293,-12.269,-10.300,-10.863,-26.321,P<0.05).Conclusion The effect of ankle arthrodesis is ideal for traumatic arthritis of the foot and ankle.
