输尿管软镜下钬激光碎石术治疗上尿路结石的临床排石率探讨

摘　　要：目的对输尿管软镜下钬激光碎石术治疗上尿路结石的临床排石率展开系统分析。方法方便选取该院2019年9月—2021年9月治疗的200例上尿路结石患者作为研究对象,随机分为对照组(n=100)、观察组(n=100)。对照组以经皮内镜取石术治疗,观察组以输尿管软镜下钬激光碎石术治疗,研究两组患者临床效果、临床排石率、并发症、炎症反应水平与免疫功能。结果治疗前,观察组与对照组炎症反应水平与免疫功能对比,差异无统计学意义(P>0.05);治疗后,研究组与对照组患者炎症反应水平对比,差异有统计学意义(t=25.541、4.707,P<0.001),研究组免疫功能显著改善,差异有统计学意义(t=4.063、4.359,P<0.001);观察组临床效果(98.00%)、临床排石率(96.00%)高于对照组(88.00%、86.00%),差异有统计学意义(χ^(2)=7.680、6.105,P<0.05)。观察组并发症发生率(3.00%)低于对照组(15.00%),差异有统计学意义(χ^(2)=8.791,P<0.05)。结论以输尿管软镜下钬激光碎石术治疗上尿路结石患者可明显提升临床效果与临床排石率,改善炎症反应水平与免疫功能,减少并发症,值得临床研究与推广。Objective To systematically analyze the clinical expulsion rate of upper urinary tract calculi treated with holmium laser lithotripsy under flexible ureteroscope.Methods Conveniently selected 200 patients with upper urinary tract calculi treated from September 2019 to September 2021 in the hospital as the research objects.They were randomly divided into control group(n=100)and observation group(n=100).The control group was treated with percutaneous endoscopic lithotripsy,and the observation group was treated with holmium laser lithotripsy under flexible ureteroscope.The clinical effect,clinical stone expulsion rate,complications,inflammatory response level and immune function of the two groups were studied.Results Before treatment,there was no statistically significant difference in the level of inflammatory response and immune function between the observation group and the control group(P>0.05).After treatment,the inflammatory response level of the study group was compared with that of the control group,the difference was statistically significant(t=25.541,4.707,P<0.001).It was significantly improved with immune function in the study group,the difference was statistically significant(t=4.063,4.359,P<0.001).The observation group clinical effect(98.00%)and clinical stone expulsion rate(96.00%)were higher than control group(88.00%,86.00%),the difference was statistically significant(χ^(2)=7.680,6.105,P<0.05).The observation group complication rate(3.00%)was lower than the control group(15.00%),the difference was statistically significant(χ^(2)=8.791,P<0.05).Conclusion Holmium laser lithotripsy under flexible ureteroscope in the treatment of patients with upper urinary tract calculi can significantly improve the clinical effect and clinical stone excretion rate,improve the level of inflammatory response and immune function,and reduce complications,which is worthy of clinical research and promotion.
