手针和不同刺激参数电针干预溃疡性结肠炎小鼠的疗效差异研究

摘　　要：目的探讨手针及不同刺激参数下电针对溃疡性结肠炎(ulcerative colitis,UC)模型小鼠的疗效差异,筛选针刺治疗UC的最佳干预方法及刺激参数。方法5%葡聚糖硫酸钠、自由饮水诱导小鼠建立UC模型,选取双侧“天枢”穴、双侧“上巨虚”穴进行针刺干预。实验1:在实验第7~11天对小鼠进行针刺干预,每次干预15分钟。空白组与模型组不给予治疗;治疗组分别为电针1组(疏密波,2/100 Hz,0.3 mA)、电针2组(疏密波,2/100 Hz,0.6 mA)、电针3组(疏密波,2/100 Hz,0.9 mA)、电针4组(连续波,100 Hz,0.9 mA)、手针组。实验2:在实验第3~11天对小鼠进行针刺干预,每次干预15分钟。空白组与模型组不给予治疗;治疗组分别为电针1组(疏密波,2/100 Hz,第3~6天:0.3 mA,第7~11天:0.9 mA)、电针2组(疏密波,2/100 Hz,0.3 mA)、电针3组(疏密波,2/100 Hz,0.9 mA)、手针组。比较各组小鼠体质量、疾病活动指数评分(disease activity index,DAI),采用苏木精—伊红染色法观察结肠组织形态,免疫组化法检测各组小鼠结肠组织内肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、髓过氧化物酶(myeloperoxidase,MPO)、基质金属蛋白酶-9(matrix metalloprotease-9,MMP9)表达水平。结果实验1中电针1组、电针3组小鼠结肠长度较模型组明显增加(均P<0.05),其中电针3组小鼠体质量较模型组显著增加(P<0.05),且结肠组织炎性浸润减轻,其他组未见明显差异。实验2中电针2组、电针3组小鼠体质量较模型组增加(P<0.01;P<0.05),两组DAI评分较模型组降低(P<0.01;P<0.05),结肠组织中TNF-α、MPO、MMP9表达下降,但电针3组小鼠结肠长度较模型组显著增加(P<0.01),结肠组织损伤较轻。电针治疗UC的疗效优于手针,疏密波疗效优于连续波,频率2/100 Hz疗效优于100 Hz,2/100 Hz下0.9 mA疗效优于0.3 mA、0.6 mA。结论针刺治疗UC小鼠的最佳干预方式为电针,最佳刺激参数为0.9 mA、2/100 Hz、疏密波。Objective To compare the therapeutic effects of manual acupuncture(MA)and electroacupuncture(EA)on ulcerative colitis(UC)model mice with different stimulation parameters,and screen the best intervention methods and stimulation parameters of acupuncture in the treatment of UC.Methods The UC model was established by intrarectally administration of 5%dextran sodium sulfate.Bilateral“Tianshu”points and“Shangjuxu”points were selected for acupuncture intervention.ExperimentⅠ:On the 7th-11th day,the mice were intervened by acupuncture for 15 minutes each time.The control group and the model group were not given treatment;the treatment groups were divided into the EA1 group(dilatational wave,2/100 Hz,0.3 mA),the EA2 group(dilatational wave,2/100 Hz,0.6 mA),the EA3 group(dilatational wave,2/100 Hz,0.9 mA),the EA4 group(continuous wave,100 Hz,0.9 mA)and the MA group.Experiment Ⅱ:On the 3th~11th day,the mice were intervened by acupuncture for 15 minutes each time.The mice in the control group and the model group were not given treatment;the treatment groups were divided into the EA1 group(dilatational wave,2/100 Hz,3th-6th day:0.3 mA,7th-11th day:0.9 mA),the EA2 group(dilatational wave,2/100 Hz,0.3 mA),the EA3 group(dilatational wave,2/100 Hz,0.9 mA)and the MA group.The body weight and disease activity index(DAI)in each group of mice were compared,the morphology of colon tissue was observed by hematoxylin eosin staining,and the expression levels of tumor necrosis factor-α,myeloperoxidase and matrix metalloprotease-9 in colon were detected by immunohistochemistry.Results In Experiment Ⅰ,the colon lengths of UC mice in the EA1 group and the EA3 group were increased significantly compared with the model group(all P<0.05),the weight of UC mice in the EA3 group was increased significantly(P<0.05),and the inflammatory infiltration of colon tissue was decreased,and there was no significant difference in other groups.In Experiment Ⅱ,the weights of UC mice in the EA2 group and the EA3 group were increased compared w
