株洲地区围绝经期女性绝经综合征患病现状及其对激素替代治疗的认知情况

摘　　要：目的调查株洲地区围绝经期女性绝经综合征(MPS)患病现状及其对激素替代治疗(HRT)的认知情况。方法采用分层整群随机抽样法,抽取株洲市386名围绝经期女性进行问卷调查,了解其MPS患病情况及其对HRT的认知情况。结果(1)386名围绝经期女性中,共诊断MPS患者283例,患病率为73.3%。(2)最常见的围绝经期症状为疲劳乏力,其次为易激动、潮热出汗、失眠、性生活障碍和骨关节痛。绝经过渡早期以潮热出汗、易激动、疲乏无力、抑郁及疑心、心悸、失眠等症状为主;绝经过渡晚期各症状的发生率均较绝经过渡早期明显升高;而绝经后期则主要以性生活障碍、骨关节痛、疲乏无力、失眠、眩晕等症状为主。(3)仅有89名(23.1%)女性对HRT有所了解;了解途径以网络、医生、亲戚朋友为主;仅有4名(1.0%)女性接受过HRT;未接受过HRT的女性中,有79.1%在科普后仍表示不愿意接受HRT,主要原因是恐惧副作用。结论株洲地区围绝经期女性MPS患病率较高,但对HRT的知晓率和接受率较低,且对HRT的抵触心理较强,主要原因在于恐惧其副作用。Objective To investigate the status quo of the prevalence of menopausal syndrome(MPS)in perimenopausal women in Zhuzhou region and their awareness of hormone replacement therapy(HRT).Methods The method of stratified cluster random sampling was used to select 386 perimenopausal women in Zhuzhou City for a questionnaire investigation.The prevalence of MPS among them as well as their awareness of HRT was understood.Results(1)Among the 386 perimenopausal women,a total of 283 patients were diagnosed with MPS,with a prevalence rate of 73.3%.(2)The most common perimenopausal symptoms were fatigue,followed by irritability,tidal fever and sweating,insomnia,sexual disorders,and arthralgia.In the early menopausal transition period,the major symptoms included tidal fever and sweating,irritability,fatigue,depression and suspiciousness,palpitation,and insomnia;the incidence rates of various symptoms in the late menopausal transition period were significantly increased than those in the early menopausal transition period;furthermore,the major symptoms in the late menopausal period included sexual disorders,arthralgia,fatigue,insomnia,and dizziness.(3)Only 89(23.1%)women knew about HRT mainly via the internet,doctors,and relatives and friends.Only four(1.0%)women had received HRT.Of women who had never undertaken HRT,79.1%,after undergoing science popularization,claimed that they were unwilling to receive HRT mainly because of fear of its side effects.Conclusion Perimenopausal women in Zhuzhou region have a relatively high prevalence rate of MPS but relatively low awareness rate and acceptance rate regarding HRT;moreover,they refuse HRT more strongly mainly because of fear of its side effects.
