中药通络蠲痹液电离子导入与关节腔冲洗治疗膝骨关节炎合并积液的临床研究

摘　　要：目的:观察中药通络蠲痹液电离子导入联合关节腔冲洗治疗膝骨关节炎合并积液的临床疗效。方法:选择2020年1月-2021年3月甘肃省酒泉市人民医院膝骨关节炎合并积液患者70例为研究对象,按治疗方法不同分为两组,对照组30例给予关节腔内冲洗,观察组40例在对照组的基础上加用中药自拟方通络蠲痹液电离子导入,共治疗3周,观察治疗前后膝关节周径及美国西安大略和麦克马斯特大学(Western Ontario and McMaster University,WOMAC)骨关节炎指数和膝骨关节炎严重性指数(Index of Severity for Osteoarthritis,ISOA)评分,检测血清炎性因子白细胞介素-17(Interleukin-17,IL-17),前列腺素E_(2)(Prostaglandin E_(2),PGE_(2))和一氧化氮(Nitric Oxide,NO)及透明质酸(Hyaluronic Acid,HA)水平,统计患者总有效率,比较临床疗效。结果:两组治疗后与本组治疗前比较,膝关节周径及WOMAC指数和ISOA评分,血清IL-17,PGE_(2)和NO指标都有所下降,HA水平升高(P<0.05),治疗后观察组上述各项评分明显低于对照组(P<0.05),总有效率较对照组明显提高(P<0.05)。结论:中药通络蠲痹液电离子导入与关节腔冲洗治疗膝骨关节炎合并积液可活血通络,消肿止痛,降低关节内压,消除滑膜炎,改善关节微循环,提高临床效果,值得推广应用。Objective:To observe the clinical effect of the Tongluo Juanbi solution(通络蠲痹液)electric iontophoresis plus intra-articular irrigation on knee osteoarthritis with effusion.Methods:70 cases from January 2020 to March 2021 were divided into 2 groups according to different treatment methods.30 cases in the control group were given intra-articular irrigation,and 40 cases in the treatment group were treated by the Tongluo Juanbi solution electric iontophoresis plus intra-articular irrigation.The treatment lasted for 3 weeks.Results:After treatment,the knee joint circumference,WOMAC osteoarthritis index score,ISOA score,and the levels of the serum IL-17,PGE_(2) and NO in the two groups were all decreased compared with before treatment,and the level of HA in the two groups were increased compared with before treatment(P<0.05),and all the above indicators in the treatment group were better than the control group(P<0.05).The total efficiency in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion:The Tongluo Juanbi solution electric iontophoresis plus intra-articular irrigation on knee osteoarthritis with effusion can promote blood circulation,relieve swelling and pain,reduce intra-articular pressure,eliminate synovitis,ameliorate microcirculation of joint,and improve the clinical effect.It is worthy of promotion.
