拨筋手法联合艾灸治疗膝关节滑膜皱襞综合征临床研究

摘　　要：目的:探讨拨筋手法联合艾灸治疗膝关节滑膜皱襞综合征的临床疗效,为临床治疗提供参考。方法:选取2017年6月-2018年9月诊治的53例膝内侧滑膜皱襞综合征患者,随机分为观察组和对照组,观察组予局部拨筋手法,同时选取病灶周围穴位行艾灸治疗配合股四头肌功能锻炼;对照组仅予拨筋手法配合股四头肌功能锻炼。结果:观察组疗效优于对照组(P <0.05),观察组利斯霍姆膝关节功能评分(Lisholm Knee Function Score,Lysholm评分)优于对照组(P <0.05),观察组疼痛视觉模拟评分法(Visual Analogue Scale,VAS)评分低于对照组(P <0.05);治疗后,观察组患者的白细胞介素-1(Interleukin-1,IL-1)和肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)水平显著优于对照组,P <0.05。结论:拨筋手法联合艾灸疗法对改善膝内侧滑膜皱襞综合征局部疼痛肿胀、膝关节功能疗效确切,并能减少其他治疗带来的痛苦,是治疗膝内侧滑膜皱襞综合征的有效方法。Objective: The clinical effect of toggling tendon manipulation plus moxibustion on knee joint synovial fold syndrome was explored in order to provide more reference for its clinical treatment. Methods: 53 cases from June 2017 to September 2018 were selected and randomly divided into the observation group and the control group. The observation group received local toggling tendon manipulation plus moxibustion at the acupoints around the lesion and quadriceps functional exercise. The control group only took toggling tendon manipulation plus quadriceps functional exercise. Results: The clinical efficacy in the observation group was better than the control group(P<0.05). The Lysholm knee function score in the observation group was better than the control group(P<0.05), and VAS score in the observation group was lower than the control group(P<0.05). After treatment, the levels of IL-1 and TNF-α in the observation group were significantly better than those in the control group, and the differences between the two groups were significant(P<0.05). Conclusion: The toggling tendon manipulation plus moxibustion on knee joint synovial fold syndrome is effective in ameliorating local pain and swelling of patients and knee joint function, can reduce the pain caused by other treatments. It is an effective method for medial knee synovial fold syndrome.
