耳穴皮内针治疗冠状动脉粥样硬化性心脏病患者失眠的疗效及对血清褪黑素、瘦素的影响

摘　　要：目的:明确耳穴皮内针治疗冠状动脉粥样硬化性心脏病(简称冠心病)患者失眠的临床疗效。方法:将在广东省第二中医院心血管科住院治疗的60例冠心病失眠患者采用随机数字表法分为对照组和治疗组,每组30例。在给予冠心病常规治疗的基础上,对照组给予阿普唑仑辅助睡眠治疗;治疗组在常规治疗后给予耳穴皮内针治疗,两组均治疗4周。分别于治疗前、治疗后对匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index,PSQI)评分、阿森斯失眠量表(Athens Insomnia Scale,AIS)评分及血清褪黑素、瘦素水平进行评价。结果:治疗4周后,两组PSQI及AIS评分均较治疗前有显著改善(P <0.05),且治疗组睡眠质量、睡眠效率、睡眠障碍及日间功能障碍评分低于对照组(P <0.05),总睡眠质量、白天身体功能、白天嗜睡评分低于对照组(P <0.05)。治疗后,治疗组血清褪黑素水平显著升高,且高于对照组,差异有统计学意义(P <0.05);瘦素水平明显降低,且低于对照组(P <0.05)。治疗组的总有效率高于对照组(P <0.05)。结论:耳穴皮内针疗法对冠心病失眠患者疗效确切,值得临床推广应用。Objective: To clarify the clinical efficacy of auricular intradermal acupuncture on insomnia in patients with coronary atherosclerotic heart disease. Methods: 60 cases were divided into the control group and the treatment group by the random number table method, with 30 cases in each group. Both groups were given conventional treatment for coronary heart disease, the control group was given alprazolam more to promote sleep, and the treatment group was given auricular intradermal acupuncture after the conventional treatment. Both groups were treated for 4 weeks. Results: After 4 weeks of treatment, the PSQI and AIS scores in the two groups were significantly ameliorated compared with before treatment(P<0.05). Compared with the control group, the treatment group gained lower scores of sleep quality, sleep efficiency, sleep disturbance, and daytime dysfunction of PSQI(P<0.05). The treatment group was lower than the control group in the scores of the overall sleep quality, daytime physical function, and daytime sleepiness of AIS(P<0.05). After treatment, the level of serum melatonin in the treatment group was significantly increased, and was higher than that in the control group, the difference was statistically significant(P<0.05);and the level of leptin in the treatment group was significantly reduced, and was lower than the control group(P<0.05). The total efficiency in the treatment group was obviously higher than the control group(P<0.05). Conclusion:Auricular intradermal acupuncture shows a definite effect on insomnia in coronary heart disease patients, and it is worthy of clinical application.
