摘 要:目的 探究产科彩超监测胎儿相关血流频谱参数在胎儿畸形筛查中的应用价值。方法 选取2020年1月—2021年1月在大连市某医院产检且分娩的150例产妇为研究对象,根据分娩后新生儿是否存在畸形将孕产妇分为健康组(n=72)和非健康组(n=78)。比较2组产妇孕中期同一时间段相关血流频谱参数,从而评估产科彩超血流频谱检查[阻力指数(RI)、收缩峰与舒张末期血流速比(S/D)和搏动指数(PI),静脉导管血流量(QDV)、脐静脉血流量(QUV)及QDV/QUV和下腔静脉反流分数]在胎儿畸形筛查中诊断价值。结果 非健康组产妇PI、S/D、RI均高于健康组,差异均有统计学意义(P<0.05)。非健康组产妇QDV、QDV/QUV和下腔静脉反流分数均高于健康组,差异均有统计学意义(P<0.05);非健康组产妇QUV低于健康组,差异有统计学意义(P<0.05)。结论 产前彩超监测胎儿相关血流频谱参数可为临床评估胎儿畸形提供一定参考依据,值得临床推广。Objective To explore the application of obstetrical color ultrasound monitoring fetal blood flow spectrum parameters and in the evaluation of fetal malformation.Methods A total of 150 pregnant women who underwent antenatal care and delivery in a hospital in Dalian from January 2020 to January 2021 were selected as the objects.According to malformation of newborns after delivery,the pregnant women were divided into healthy group(n=72) and unhealthy group(n=78).The related blood flow spectrum parameters at the same time in the second trimester of pregnancy were compared between the two groups,so as to evaluate the diagnostic value of obstetrical color ultrasound blood flow spectrum examination(resistance index(RI),systolic peak to end diastolic blood flow velocity(S/D) and pulsatility index(PI),ductus venosus(QDV),umbilical vein(QUV),QDV/QUV and inferior vena cava reflux fraction) in fetal malformations.Results The levels of PI,S/D and RI in the non-healthy group were significantly higher than those in the healthy group and the differences were statistically significant(P<0.05).The scores of QDV,QDV/QUV and inferior vena cava reflux in the non-healthy group were significantly higher than those in the healthy group,and the differences were statistically significant(P<0.05).The QUV in the non-healthy group was significantly lower than that in the healthy group,and the differences were statistically significant(P<0.05).Conclusion There is a certain correlation between prenatal color ultrasound monitoring of fetal blood flow spectrum parameters,which can provide a reference basis for clinical evaluation of fetal malformations.It is worthy of clinical promotion.
