来曲唑结合人绝经期促性腺激素对PCOS不孕症患者疗效的影响

摘　　要：目的探讨在多囊卵巢综合征(PCOS)合并不孕症患者治疗中,联合应用来曲唑与人绝经期促性腺激素(HMG)展开治疗干预的价值。方法回顾性选取2016年1月-2020年12月内在该院就诊的PCOS患者288例,以随机数表法,分为来曲唑结合人绝经期促性腺激素治疗组(n=144)与克罗米芬结合人绝经期促性腺激素治疗组(n=144),统计用药后疗效。结果来曲唑结合人绝经期促性腺激素治疗组卵泡成熟天数低于克罗米芬结合人绝经期促性腺激素治疗组,差异有统计学意义(P<0.05);来曲唑结合人绝经期促性腺激素治疗组HCG日内膜厚度高于克罗米芬结合人绝经期促性腺激素治疗组,差异有统计学意义(P<0.05);来曲唑结合人绝经期促性腺激素治疗组治疗后E2指标为(165.7±10.1)pmol/L、FSH为(6.0±1.1)U/L、LH为(5.4±1.5)U/L,低于克罗米芬结合人绝经期促性腺激素治疗组(181.2±8.0)pmol/L、(6.8±1.3)U/L、(7.1±1.2)U/L,差异有统计学意义(t=14.436、5.637、10.620,P<0.05);来曲唑结合人绝经期促性腺激素治疗组妊娠率优于克罗米芬结合人绝经期促性腺激素治疗组,差异有统计学意义(P<0.05);来曲唑结合人绝经期促性腺激素治疗组治疗期间不良反应总发生率比克罗米芬结合人绝经期促性腺激素治疗组低,差异有统计学意义(P<0.05)。结论临床在治疗PCOS并发不孕症患者时,以来曲唑与人绝经期促性腺激素实施联合治疗干预,可有效改善促排卵能力,提升患者成功妊娠率的同时,还具有较高安全性,适宜深入研究与推广。Objective To explore the value of combined application of letrozole and human menopausal gonadotropin(HMG)in the treatment of patients with polycystic ovary syndrome(PCOS)and infertility.Methods Retrospective analysis 288 PCOS patients who were in the hospital from January 2016 to December 2020.Divided into letrozole combined with human menopausal gonadotropin therapy group(n=144)and clomiphene combined with human menopausal gonadotropin therapy group(n=144)by random number table method,and the efficacy of the drug was calculated.Results The number of follicular maturation days in the letrozole combined with human menopausal gonadotropin treatment group was lower than that in the clomiphene combined with human menopausal gonadotropin treatment group,the difference was statistically significant(P<0.05).The HCG daily intimal thickness in the letrozole combined with human menopausal gonadotropin treatment group was higher than that in the clomiphene combined with human menopausal gonadotropin treatment group,the difference was statistically significant(P<0.05).After treatment in the letrozole combined with human menopausal gonadotropin treatment group,the E2 index was(165.7±10.1)pmol/L,FSH was(6.0±1.1)U/L,and LH was(5.4±1.5)U/L,lower than the clomiphene combined with human menopausal gonadotropin treatment group(181.2±8.0)pmol/L,(6.8±1.3)U/L,(7.1±1.2)U/L,the difference was statistically significant(t=14.436,5.637,10.620,P<0.05).The clinical pregnancy rate of letrozole combined with human menopausal gonadotropin treatment group was better than clomiphene combined with human menopausal gonadotropin treatment group,the difference was statistically significant(P<0.05).The total incidence of adverse reactions during the treatment of letrozole combined with human menopausal gonadotropin treatment group was lower than that of clomiphene combined with human menopausal gonadotropin treatment group,the difference was statistically significant(P<0.05).Conclusion In the clinical treatment of patients with PCOS complicated
